现将自己preliminary surgery 经历的点滴与这些同学分享。
Preliminary surgery 是条曲折的路，是没有搭上到山顶的直通车的后备选择，但如果
把握好，一样可以最终引上大路，到达目的地。
我开始prelim前，听到的都是关于prelim surgery 魔鬼训练的描述：早晨4点多起床，
查房时昏昏欲睡，每天只能睡4-5小时……现在回首我的prelim surgery一年：劳累，
但却从容。除非on call，我基本保证每天睡8小时，并长了10磅体重。
Prelim surgery的疲劳来自3方面：日常工作的劳动强度、陌生医疗体系带来的精神压
力，和应对第二次match的心理压力。
Prelim surgery劳动强度比其他专业要大，但不像传说的那么离谱，尤其现在有80h/
week的duty hour限制，我们比老师辈的人好多了。劳动强度在不同program不尽相同，同一
program不同的轮转医院或科室也不一样。牛校和大的program劳动强度更大 |

第一个月在VA的CT surgery，第一天是6月24日星期天，PGY-2这天起休息一周，到7月1
日才开始重新上工。整个service 5个attending (4个cardiac，1个thoracic)，一个
fellow，两个intern。病人一般是术前1-2天住院，术后一般还是intubated，直接进
Surgical ICU，几天后去PCU (step-down unit, 偶尔有病人去Ward，就是普通病房）
，然后去rehab, nursing home或者直接回家。
每天早上4-5点到，我们两个intern，每人管5个病人。我的co-intern抽签抽到patient
list的前一半，全是SICU病人，我抽到后一半，1个SICU，4个PCU病人。6点跟fellow
查房，基本就是电脑前的sitting rounds，intern汇报，presentation格式比较简单，

近视的人挺多的，确实很让人烦恼。这些都是我的个人经验，我觉得很多人知道lasik
surgery，但可能主要因为害怕和不了解而没有去考虑过，所以我觉得share一下我的经
验，可以让更多人去试图更加了解这个option。
戴眼镜的弊端
戴眼镜影响外观，总显得比较nerdy，如果要去个什么场合，穿套漂亮的裙子，化个妆
，但脸上还戴着这么副眼镜，真是很多余，很不搭配。眼镜还影响气质，很多时候脸上
出油出汗眼镜就往下滑，不是总要用手去扶一下，就是必须抬头看东西，把人显得傻傻
笨笨的，原本潇潇洒洒个人，顿时没了那种帅气的风度。影响视角范围，不能随意上下
左右随便看，总得更多带上头一块转，我觉得长期戴眼镜的人突然取下眼镜，眼神总是
怪怪的，无神发直，原因可能就是在此吧。长期戴眼镜还会在鼻梁上留下一道印子，很

emergency surgery means it has to be done now, like a ruptured viscus,
necrotizing fasciitis, things in that nature that if no surgery is performed
right away the patient might die,so no matter if it's in the middle of the

看了你的描述，醫生的助手已經替你打了電話了等等，已經表明接受了你的case，只是

没见过病人complain，估计注射前都打过versed或者fentanyl了。
很多gen surg program都没有cardiac surgery rotation，除非是elective，我听不止
一个人说过。我这个program除了PGY-1，以后就没有cardiac surgery了，thoracic
surgery倒是有。

发帖数: 577 | 36 第二周同样是从星期天开始，手术室开始恢复正常了，虽然有7月4日，但是这一周还是
有三天手术，多了4个新病人。从上一个intern“继承”下来的10个老病人，在我们两
个intern 磕磕绊绊的努力下，终于在星期二之前全部出院了。Intern的一个主要任务
就是送病人出院，这点内科外科没有区别。
Fellow也换了人，上一个fellow跑到另一个医院又开始做一个fellowship。我们这里CT
fellow的任务就是指挥intern，参加所有的手术，以及安排新的手术，每个星期只有
周六晚上才算off。新的fellow说话极快，一分钟可以吩咐20件事情，我们两个intern
只能赶紧记下来，回workroom对照一下，然后赶快把医嘱写了。他记性极好，白天如果
在楼道里碰到了，会问早上吩咐过的事情做得如何了，比如上午的CT scan结果出来没
有，swallow study的正式报告上怎么写的等等。
7月4日那天轮到我值班，我的co-intern不上工，除了CT surgery之外，从10AM-8PM，

room去，说已经和另一个医生约好时间，我去了一个上午就可以搞定了。第二天按照
医生说的，去做了手术。现在保险公司来了账单，说我做的手术是在emergency room做
的，属于emergency service，不属于day surgery.这样一来，我就要自己掏很多钱。
labor and delivery room是24小时开放的，我不知道怎么医生会让我去那儿，当时也
没有和我说去那边是emergency,现在我这样可以argue吗？明明预约的，怎么就说成是
emergency了呢？当时医生说可以过两周再做，我问医生有什么区别吗，医生说没有区
别，我才去做的。实在是比较穷，一下要多掏几千刀，太郁闷了，求问大家的建议，谢

Authorization 才可以做的。前不久我就做過一個手術，我的primary doctor 寄給我
referral authorization, 我拿著這封信才可以去看specialty physician,才可約手術
時間的。
這封信清楚的寫道：

医院，解释了一下这个是scheduled surgery.医院说要superviser同意把这个

是，我的ob不是给我做手术的人，这样我的primary doctor到底是我的ob还是给我做手

发帖数: 59 | 42 有的醫療網對于女性去看婦產科醫生是可以直接去看的，不需要轉診。你是不是直接去
看婦產科醫生的？婦產科醫生都會做手術的，通常是你看的這個婦產科醫生就是給你做
手術的醫生，不可能自己的病人讓別的醫生給做手術，同樣是醫生讓別人做手術是顯示
自己醫術不高明？你這手術是最普通的手術了，又不是什么疑難病，真奇怪？！
anyway，就算是他/她治不了你的病轉給其他醫生，也要有書面的轉診信的，否則是不
和程序的、不合法的。如果真的沒有這封信的話，那你就不用擔心，你是贏家。
至于你的那些病例資料你問醫生要才會給你的。
但是，有關billing paper（比如：每次看醫生的co-pay;surgery co-pay等等)當時交
過錢之后當場給你receipt的。
現在你要做的是，去找那個口頭上說讓你第二天做手術的醫生要referral letter,如果
沒有，那么問題就出在這里了，打官司你也贏了。 |

unvailable for an earlier date.
Is there any suggestion in persuading the surgeon to schedule my surgery
