偶然读起，发现，其实我们很多人的父母辈都是某种意义上的holocaust survivors 啊
。他们因为时代和历史的原因，被剥夺，扭曲的环境中成长起来，然后他们的生存机制
(survive mechanism)又在他们自己建立的家庭里影响到下一代的成长，造成trauma
transmission.
读到里面一个个Holocaust Survivors的下一代孩子的述说，痛惜万分，从他们身上，
我能看到很多中国孩子（6，7十年代出生）的影子。
作者令人信服的说明Narcissistic family是一个很好的model来分析这种创伤的在

I would say, if she hadn't been through her mother's suicide when she was 12,
or her Father was caring instead of aloof and selfish, she would have much
more smooth life.
I would say her fight towards the outside world, no matter how splendid,
attests her lack of sense of belongings and secruity.

到了医院，急诊室门口六辆警车一字排开。拍了下脑门，居然忘记了满月之夜？
一路飞奔到ICU。 ICU静悄悄的，全然没有交接班前的热闹场面。夜班任务分配单挂在
墙上，6个病人，3个护士，一人两个。交接班的护士告诉我，两个病人都很简单，是
Trauma，一个是车祸，一个是从自己家的楼梯上摔下来。
车祸的女孩我是知道的，三天前 我上班时进病房的，因为车祸造成了颈椎的不稳定和
C2的骨折。病人刚到病房，脑神经手术医生已经等在那里准备给她装Halo。女孩一看到
Halo就开始声嘶力竭的哭喊……另一层楼的护士给ICU打电话抱怨，你们ICU的护士真会
折磨病人。
交接班结束后，我例行公事地查房。车祸的女孩正对着镜子往她的Halo上缠花绳，看到
我，浅笑着问，“好看吗？”我心想，“得，这位小姐可以转到其他病房了。”
另外一个病人安静地平躺在床上，除了眼睛随着我位置移动而从左边转到右边又从右边
转到左边，其他身体部位都是僵硬的。从头查到脚，只看到了一处后背瘀青。和病人聊
了聊，她一个人在家，不小心从拐角楼梯上摔下来，躺在地上有大约十个小时，直到邻

Match的结果终于尘埃落定，周1等电邮的时候非常紧张，因为成败就在此了，虽然一直
告诉自己，今年只是热热身，成败都可以接受，但到真的揭晓的时候，还是很紧张，毕
竟一旦卷入其中就身不由己，何况自己也还是有所付出。到周4等match program的时候
虽然也有点紧张，但程度已经大大减少，1个原因是毕竟match上才是头等大事，去哪里
尚在其次，还有1个原因是心理还是有所准备了，虽然希望留在home school，但直觉告
诉我恐怕是非常困难了，在我rank list上第二位的program应该就是我要去的地方，果
不其然。经过几天的等待，发现果然是要搬家了。

我自从知道有“为人父母",就想来推荐此书，但是一再犹豫。因为收录的这下案例非常
非常的extreme。有幼童亲见母亲被杀，或是遭遇性侵，还有冷血少年杀手，遭邪教洗
脑的孩子，和如书名所诉，被和狗一起圈养的孤儿。这绝对不是一个“容易”读的书。
我一直认为我的心里承受能力很不错，但是两次读此书，我都经常要take a break，我

不知放哪儿让我感觉舒服点儿，借人气暂且放这儿吧。
孩子被带走3个多月，从冬天到夏天。。。他们都是无辜的天真活波可爱的孩子。没用
的妈妈不是那种只生不养，no responsibility 的妈妈，而是双方家人没有人能帮忙，
自己又太爱孩子，总想让这些孩子有个体面的生活，自己天资不足，又不能同时handle
很多事，压力太大，一时判断失误，做了件现在想来是件stupid的事情，(其实孩子进
家里，妈妈粗心，心急立马去学校找，5 minutes回家发现孩子,在警察到学校之前, 警
察作了记录这事结束了)2小时后DCF带走了所有的孩子（第二次）。
请那些天赋很强，上帝又赐予您多方面能力的人能够出谋划策，Bring kids back to
home。能感觉孩子们在精神上像我一样在崩溃的边缘，我已3次因心脏问题进emergency

了，难得空闲。在Labor Day假期时再写下过去4个月的OB经历，给自己一个了解，也给
考板的战友们一点借鉴。
OB并不如有些朋友想象的那么容易找。现在越来越多的教学医院不再接受OB的申请，
private practice则更没有动力来让人来shadow。如果打算走正规渠道的，建议及早申
请，不要等到CK/CS考完、申请即将开始之前才行动。至于非正规渠道，则是八仙过海
，各显神通，但也要及早积累关系。与我经历而言，四处碰壁之后，终于在一个
community hospital ED、一个free clinic、和一个university hospital ED做了三个
OB，在下文中一一道来。我打算申请Emergency Medicine，所以主要精力都花在ED和

章很麻烦，耽误了2个多月，材料才得以从学校寄出。个人觉得申请算是比较顺利。把
我当时准备的情况跟大家说一下也许对将要申请的人有一点帮助。
我个人从这里寄出去的材料包括：
1.国内护士执照的公正，在这里网上找的公证处，在纽约，叫最佳公正，电话

2.指纹卡，就在我住的地方的警察局办的，free
3.handbook上的申请表，贴一张照片在右下角（handbook链接：http://www.rn.ca.gov/pdfs/applicants/exam-app.pdf）
这一页有一项是填社会安全号，我当时没有就空着的，没有关系。
4.两张支票，一张办指纹的($51)，一张申请的($75)

费用低廉，只有20%左右。能处理一般伤害和急病，比如划破，哮喘，发烧等等。
emergency法律规定不得先收费看病，所以是非法移民和
无保险者的首选，但常常要等很长时间。
emergency的使用还是要看是否有生命危险，最主要的
是trauma center， 而且分级，进了emergency，护士先会预先
看一下，如果达不到trauma level，你就只能坐等，因为医院
也知道大部分去emergency又达不到trauma标准的，都是收不到钱的。 |

加州非法移民保守估计占加州总人口10%，实际上远远不止，因为他们能生。Orange
County's Santa Ana, 50% of its residents are illegal immigrants.
1. 洛杉矶的公立医院，非法移民进急诊一样全力抢救，住的最久的一个病人，八年。
要多少钱？？公立医院的一个非法移民住院一个月就要150万美金！！所以医院宁可租
一个飞机也要专门把病人送回他的家乡！洛杉矶一年花五十亿美金在非法移民的医疗上
面。洛杉矶交通罚单为什么动辄几百美金，其他的州才几十美金？交通罚单的20%直接
就是给非法移民看病付账单的。非法移民进医院还不说英语，骄傲的说 SPANISH ONLY。

至心理上为了自卫而故意遗忘，一种身体自卫的机制 -- 好些电影电视都以此为主题展
开一段悬疑剧，某人小时候受了 trauma，但基于自卫而被遗忘，然后慢慢经过治疗或
又一次的 trauma 而回忆起来。。。。
心理是个很复杂的事情，你绝不能拿一个人的反应就说所有人都应该这样。 |

将一件小事无限夸大，然后用夸大情形中的小概率事件作为不提供对小trauma的任何记

1. 关于病情
如果3号车祸到今天没有醒来,需要区分几种情况.如果是从来都没有醒来过,则是非常不
好的现象,虽然也有二十几天后或更长时间醒来的情况,但是brain trauma一般是昏迷时
间越长情况越凶险.如果在过去的几天里,医生能让他醒来但故意控制他在昏迷状态则是
好现象,他醒来的可能性非常大. 对于严重的brain trauma,即使病人能醒来,医生也会
控制他醒来的时间,因为马上醒来脑压急升也会致命.你们需要问一下主治的脑科医生他
是哪种情况,他脑部是否有严重出血?昏迷时脑压是否稳定? 从你讲的情况看,他唯一的
乐观现象是,他已经挺过72小时,brain trauma最危险的时间.
2. 关于帮助
可以求助学校,学生会,当地的教会组织,但是这个帮助对昏迷的病人并无直接作用,主要
是对他亲属的帮助.伤者只有寄托给医生和上帝,你们现 |

有没有心理学家论证，说，这种trauma 如果真的被block 了，要把它解放出来，必须
去面对他，才算是健康？不然可能会变态的？
