有NIJ LEVEL的防弹衣， 不管是硬的还是软的， 都已经包括了backface deformation
的要求（大概是不超过一寸半）。 level IIA能防止12 gauge 00 buck的弹丸的穿透，
也能防止该弹药的blunt force trauma对人造成重伤， 把防弹衣backface
deformation限制在一寸半以内。
backface deformation是用泥巴来测试的。 人体比泥巴更柔韧， 泥巴上的一寸半深的
坑在人体上则不会这么严重。 穿不同等级的防弹衣挨了12 gauge #00 buck后的感受从
轻轻拍打到大锤砸胸都有可能； 但即便是最严重的情况， 一发霰弹也不会把人打倒打
晕， 所以根本不是自卫能依赖的东西。 |

It’s hard in the beginning, kind of clueless for me. But you will be able
to get the idea pretty quickly after 5-6 cases in computer. Here’s my
thoughts how to manage it quickly and try not to miss things.

ER的重症区锻炼一段时间的话，我觉得你的critical care skill尤其time management
只会更好，知识面也广。但其实反过来想一想，真的ICU和ER才有好出路吗？Psych NP
也不少钱哦。而且很多科室各有各的好处，真是见仁见智的选择。
最后，别担心GPA和口语。我工作的level 3 trauma center特别喜欢男生。我知道我说
这个话有人不爱听，可我没撒谎。 |

护士的大把大把，竞争相当激烈，落选的也大有人在，虽然有些学校说没有ICU的经验
也无所谓，在我看来是幌子。 无非想打捞申请费，望眼看去，录取的都是有好几年ICU
经验的， 都来自一些州的大学医学院， level 1 trauma的大医院，我们这里不谈
florida某犄角旮旯的 CRNA 学校。
ICU，medsurg 的分级是想当严格的，ICU是critical care， medsurg是acute care，
不一样的，医院不同又时这条界线会模糊，但是过来人都知道，界限非常明确。 建议
要看职业规划， 其实说句心理话， 包括我在内， 国内过来的， 20 30岁在这里上个
护士，但是语言这关再好， 也过不了那一关。 ICU找人， 想来凤毛麟角，谁会刚毕业
就进， 我们都明白。
楼主，我们撵转来到美国，弯路没少走，亏没少吃，学费没少付。多干一两medsurg，
不会耽误你大事。 心里话如果就算进level 1 trauma的大医院的medsurg, 一两年不一

。好多时候都是用clinicaltrial.gov上的临床试验做依据。还有就是同行间的交流很
重要，有时候甚至是learn from patient,有时候真的是逼到那一步了没有办法。用的
时候大家都是紧张的不得了
。正因为这样，药剂师能发挥作用的地方很多很多，成就感也很强。不过做儿科确实有
时候心里觉得很难过,不管是工作多少年的药剂师，还是我们这些新的住院，总会有几
次不愿意面对的现实。两个月前在ED做trauma bay code,有两个车祸的孩子同时送过来
，我和我的preceptor一人负责一间trauma bay.最后一个孩子没有救过来。我和
preceptor两个人抱头痛哭。还有几次，follow了很久的病人，最后坚持不下去了，每
天查房，和病人的父母已经很熟，每天看着他们受煎熬，希望和失望交替着，最后选择
放弃治疗。 |

有这么详细的回复， 谢谢！
时间过得好快， 发帖的时候我还在学习， 现在已经接近毕业了， 昨天刚把实习的
paper work交了， 这是毕业前最后一个事。
最近开始找工作， 不怎么乐观。 澳洲还是对psychologist的认可比较多， 因为政府
有给补助， 所以看psychologist基本不用自己付钱 （但是一年最多给10个免费的
session)， 看counsellor就要自己掏钱， 说是以后也有机会拿到政府的补助， 那就
慢慢看吧。 （其实我觉得除了没有权利诊断精神疾病， counsellor和psychologist做
的基本一样， 就是使用的model会有不同。）
我觉得自己开practice为时尚早， 感觉还没到那个本事， 回头再把名字砸了。打工吧
， 现在基本就是在trauma counselling (however, need further training, so wont

另外，我记得学过的是，这种persistent VS一般观察一年，一年后没大的好转的话，
算permanent VS，届时，如果不是外伤(trauma)引起的，则几乎肯定没有好转的希望了
。如果是trauma引起的，则病理上可以是因为脑灰质脑白质界面上的axons被切断，
neurons本身未受损害，目前已知只有这种情况可以象urban legend传说那样的很多年
以后好转。记的对不对，请楼上两位指正。 |

According to previous studies, there is no evidence to support the use of
steroids in the management of spinal cord injury. Administration of high
dose steroids to trauma patients can have significant adverse effects on
patient outcome. There is evidence to suggest a worse outcome in patients
with penetrating injury who receive steroids.

发帖数: 53 | 47 上个礼拜，我差点就自杀了。结果一群孩子救了我。我觉得自己不可以在他们面前自杀，否则的话，我就成了他们心中的trauma。记得心理医生第一次见我，就问我，你的生命中有没有trauma。我已经这么惹人恶心了，怎么还能去伤害这些孩子幼小的心灵呢？
然后我就重新回到校医那里。说着说着，我就泪流满面了。校医建议我休学一段时间。我说不要，休学是loser的行为。校医很担心我，说要我一定答应她，如果再有自杀的念头，就打学校on call的电话。否则，you need to be hospitalized. 我又哭了。说，why you wanna get me hospitalized?
Are you implying that I am ill? I am not. 她当时估计被我的语气愣住了，安慰了我一阵
子，和我约好明天见面。

今天第一次看therapist。那个女人可真大个儿，又高又胖，好像一片云。很难想象这
么个形象的人心理这么健康，还能给别人看病，而且日程满满。一个小时说了不少事情
，用人家的语言归纳，我在美国的第一年very traumatic。美国人和中国人对什么是
trauma的定义估计不一样。或者是我自己的定义和别人不一样，反正我是过了这么多年
才突然觉得很trauma的。
昨天晚上很想用刀子切自己一下，而且难得老公不在家，我老爹已经带我儿子上楼睡觉
了。我准备好刀，酒精棉球和创可贴，好像要动手术一样。心想，这次怎么也要真的见
血，不要老是光想想。谁知道，才在胳膊上随便练习了两下，都没来得及用劲儿，就听
见我爸叫我去给我儿子换尿布，说突然拉臭臭了。之后忙活了半天，就找不到感觉了，
以后哪天有情绪再说吧。不过那两下练习也已经留了两条很红的痕迹，现在还没有消退
的意思，也够得上self harm的定义了，没算白忙活。
