thyroidectomy，冷冻切片符合papillary，但是是diffuse sclerosing variant比较
aggressive，而且是moderately differentiated，所以当时已经知道复发的可能性很
大。七月份做了RAI，剂量是100 millicuries。RAI后的全身PET scan没有发现I-131
uptake，TSH在0.1左右，但是因为thyroglobulin antibody (TgAb)非常高（>3000），

limited. If have very obvious hyperthyrotoxic symptoms, can take propranolol

aggressive tumor. I agree with againstwind, your wife needs biopsy of the
enlarged lymph node. She can get a PET/CT before biopsy to see whether the
lymph nodes have FDG uptake. Should these tests confirm recurrent disease,
it suggests that the tumor is radio-I insensitive. If PET/CT only shows
localized disease in the neck, surgery need to be considered. If she has

LEVOTHYROXINE， TSH 很稳定保持在2以下。
2012年9月检查TSH 1.11. 10月怀孕，孕36天检查TSH 2.8. OB说TSH因为怀孕升高正常
，不需要调整。 孕12周要求再查，医生很不情愿开了单子，并告知2-3星期以后再查，
因为他们每个TRIMESTER 只能做一次甲功验血。孕14周检查TSH7.77, T3 UPTAKE 20%.
医生不负责，我自己也大意，现在后悔也晚了。我这种情况对胎儿会不会有影响？13周
做B超时貌似胎儿大小正常，符合周数。这个宝宝我们试了快3年才有的，很珍惜。
我自己没有甲减症状。
我还需要做什么检查吗？是否需要去见内分泌专科医生？
在美国，不生病还好，除了问题再碰上照本宣科，不负责的大夫真的很郁闷。有没有情
况相似的JMS,求建议，谢谢！ |

' disease , but not these two antibodies. If this antibody is positive, then
pretty much Graves disease. If negative, you still need a radioactive

“甲状腺全切&淋巴结清扫+I131放疗+TSH抑制疗法 ”三部曲治愈后，一直都有去医院
定期复查，一直都没有问题的，可是前天去复查的时候，发现有转移的情况，似乎很严
重：6个淋巴结高度怀疑是转移。不知道之前复查为什么一直没有，就是这最近一次就
转移了。而且好像转移后的癌细胞不uptake I131了，真是失望至极。
经过协和医苑专家会诊，有两个新的情况：1、专家认定这种情况还不属于失分化癌，
因为还不确定是否吸碘。2、6个淋巴结高度怀疑是转移，外科医生建议马上手术清除，
但有两个后果，手术次数越多难度越大，并发症越多，且手术根本无法彻底清除。 核
医学专家建议我继续I131，有办法让其继续摄碘治疗，以缓解症状。 朋友准备听核医
学专家的建议，采取保守治疗。
朋友希望我帮忙关注现在美国关于甲状腺乳头状癌最先进的治疗方案。特别是我现在经
过“甲状腺全切&淋巴结清扫+I131放疗+TSH抑制疗法 ”三部曲治愈后复发转移的情况。

这个方法，主要是在推算carb loading in 12 hours before the race.
还有就是要是VO2max 很高的话 （>65），这个 “Aggressive Best” 是不准的
