就像随着年龄增长皮肤会长皱纹一样，各个器官也会长一些不会对身体带来实质危害的
小东西，如果不刻意检查一辈子可能都不会知道，我们叫它“没有临床意义”。有时甚
至为了把这些东西和不太好的恶性病变区分出来还不得不做有创检查，从而给患者带来
风险(比如“开胸验肺”，别看是新闻其实在实际医疗实践中并不少见)。
所以原则上任何检查都应该是有原因才考虑去做的，也就是如果如果结果异常能通过及
时的干预使患者获益。但是非常遗憾的是，因为多种原因所致，国内很热衷给病人开大
量没有必要的检查，从而带来很多没有必要的担心。
你提到的这几个检查是很有必要的
1. 糖化血红蛋白：虽然这个值在健康人群中的正常值应该低于6.5，但是因为糖尿病患
者对调节功能变差（特别你爸爸胰岛功能几乎丧失），过低的糖化血红蛋白可能让患者
出现低血糖，从而反而对患者带来伤害。目前认为像你父亲这样的老年人，最好糖化血
红蛋白控制在大于或等于7左右，也就是说可以糖尿病的用药可以"宽松"一点。（参考

满35周。
我和娃状态都不错，出血量也小，cbc都正常，医学权威信息（uptodate，我ob给我看
的）上推荐此种情况为了母亲安全要在36－37周剖，一般医学意义37周才足月，我本来
约38周剖的，bb size一直正常，不大不小。
没有护理早产的经验，上一个娃是足月，养的非常粗，这个估计不行，很希望得到些建
议，相关文章在各个书里都一笔带过，有什么特别的书推荐，或者大家觉得需要格外注
意的某个地方吗？

他拿来做凭据的是权威的医学期刊uptodate上写的，建议是36－37周之间。

即使做最坏的打算，刚才简单看了一下biliary atresia的治疗方法。前面也有MM提到
了成功的例子。这些例子很多。LZ请记住：
1）现在是20-30年以后，宝宝是在美国，手术方式和预后都有很大程度的提高。
2）Kasai手术法/Hepatoportoenterostomy，将胆道和小肠连接起来，通过那些还通畅
的小的胆道把胆汁流到小肠。
3）做最坏打算，肝移植。虽然这是很艰难的一条路，但是并不是意味着救不了宝宝。
宝宝仍然会有很大机会！这里移植、移植后的治疗都是先进的。最重要的是不能让宝宝
黄疸水平太高时间太长。所以早期发现、早期治疗就是给宝宝最大的机会。
我说这些，并不想缩小你现在经历的痛苦，现在最重要的是，用知识武装自己，配合医
生，给宝宝最好的诊断和治疗。尽快住院检查，尽快找出原因。诊断过程包括超声

流，医院所有的主任们都抱着笔记本，像小学生一样认真听，问很多好问题。
其实主要的问题还是医学学习资源，英语水平，以及住院医培训制度跟人家相差太大了
。 美国这里那个犄角旮旯的医院都随时可以查uptodate以及各种数据库，去寻找自己
临床遇到的问题的答案。 |

。家里有一个1岁两个月的女狗（spayed, 品种： beagle， 30lb）需要人照顾，因为
怕签证被check， 所以往返回国的机票买了30天， 这么长时间把狗狗boarding很是不
放心， 狗狗从小都是在家放养，pet hotel却是长时间关笼子。去年狗狗boarding过一
次5天，回家后一整周都很不安。
如果有喜欢狗狗的好心人愿意帮忙照顾狗狗， 我就能放心的回国去。狗狗一天要尿4次
， 早上起床， 中午，晚6点， 11点睡觉前。（我家开着阳台门， 中午和晚上她自己
会去阳台拉，每次带出去遛弯也会拉干净才回来。） 一天吃两餐，上班前， 下班后。
一天睡觉时间是15小时（11pm-8am，12pm-5pm）。最好一天能溜两次，她一般表现都很
好， 但是看到松鼠， 猫狗就想追。在家自己叼着网球能玩好一会。我家狗狗
对其他狗非常友好，老做人家的跟屁狗。 但是对猫就不怎么样，不过她从小就不会打
架。另外，我不希望她被大狗欺负。

是造Capsaicin的原料。好处还不少，有抗炎止痛， 促进心血管循环，促进消化。我也
是到了美国才发现，从国内来的几乎没有不吃辣的（除了我），以前我室友， 湖南来
的， 吃饭就是西兰花加上老干妈。好处很多，当然如果有消化道溃疡的要当心，而且
也会可能对其他药物有干扰，比如抗高血压药，因为会产生catecholamine儿茶酚胺.有
兴趣的可以看看。

jolla 可以看水獭，好像现在没有了，走啊走，然后,mission bay，接着走。中午去
convoy吃中餐。下午去corolado island 我很喜欢的地方，看梦露排电影的地方，沿着
海滩走走，看看婚礼。旅馆小店有家冰激凌店，点一个banana split坐下屏海临风甚爽
。然后可以往军事基地那边开开，或者去游人去的seaport village买小东西。
sea would,san diego zoo都要一天时间。wild animal park也不错，san diego到处都
很busy难得看到那么空旷的地方。
