以前在美国开了3年，一直遵纪守法，从未吃过罚单。但这次遇到个严格的警察。在州
内高速77上面经过一个小镇时，70的限速变为55，我减为55，然后大概100米内变成45
，我看到45的牌子，踩了刹车，正在减速，一个警察就藏在45限速牌很近的地方，把我
拦住，说我是52。这个警察很可恶，因为我已经踩了刹车在减速了，但需要点时间降到
45。
另外，警察说我半年前搬了家，驾照上面的地址没有改，2个罪过，让我10天内给法官
打电话，法官会告诉我罚多少。
我知道可能一要交罚款，二要上教规的课。但担心将来有了超速的纪录，保险费用会长。
我知道可以上庭辩论，但那个小镇好几个小时外，没时间再去，而且万一辩护输了
，不但要交罚款，似乎也有了超速的纪录。我知道有的州有专门做这种生意的律师，交

triage给的诊断是"red eye".叫近来一看,小孩眼睛好好的,一点都不红. 问了才知道原
来是曾经有过pink eye,现在来要医生条子好让孩子继续去daycare.这到没什么了,但她
要我开条子证明她前几天在家休息是因为孩子生病.我当然不能开了,我怎么知道她前几
天做什么.和attending汇报完后,attending和妈说了两句,这妈又向我的attending要求
开这条.我的attending真会让我开这条给她.不行,这不是让我的信誉扫地吗.马上追出
去,和attending说我不知道的事不能开条,我只能说我今天在急诊间见到她.attending
可能想想也对,就回去和那妈说不能给你开这样的条.然后她又问我要免费的体温计.当
我这是难民所啊!马上说没有,从来没给过.然后再装模作样去护士那儿再度证实这儿从
不给体温计.她很不高兴的说她可以去向她的儿科医生要. whatever. 最后又要我帮她
复印一张纸,当我是医生还是她的下属.马上说这种事不用我做,外面还有一堆病人要看,

fellowship基本在ICU和EEG两个之间考虑。我和ICU fellow讨论的时候，问他life
style的问题。他反问我，你看那些七，八十岁的surgeon，走路都不利索了，可是还是
在做手术，为什么？Because they love it.你现在首先考虑的是passion，然后才是
life style，job market之类的事情。然后对我说，我见过的中国人，就没有一个不做
电生理的。说的我那叫一个不好意思。
然后我和ICU的attending聊天，说起life style的问题，他说life style当然差了，天
天半夜被叫起来，EEG attending一年能被叫起来一次嘛？
等我和EEG attending聊天，转述了一下ICU attending说的。EEG attending说，ICU上
班归上班，不值班的时候也不用出门诊，想干啥干啥，我觉得和他换一下也不错。:P
所以说，有的时候某些名声在外的专科也没有你想得那么糟糕，找工作的时候，各个

发帖数: 810 | 10 说是两周，其实一周就一天半。带我的attending是神经肌肉疾病的专家，平时每周一
看转诊过来的新病人，周三下午看老病人。其余时间做肌电图和其他事情。这个科室是
attending轮流负责病房的service,不知道有没有attending是专职在病房的，反正我看
到schedule是大家轮流去病房。
去之前他跟我说除了白大褂什么都不要带，也不用记笔记。周一上午还有PG2的
residency来跟他一起看。基本是PG2先去问病史做体检，我就跟在旁边看。第一天实实
在在是个影子，除了跟病人握手和拼命挤出僵硬的微笑外，我就坐在旁边。PG2看完病
人出来后跟attending汇报病史讨论一下DD。不知道是因为忙还是因为这个attending本
身也是FMG，以前受的也是填鸭式的教育，他总是听完PG2的汇报后就开始谈自己的DD和
治疗方案，说完后才想起来问PG-2有没有什么补充的。也从不主动问PG2和我有没有什
么问题。我只好见缝插针的问少数问题，为了省时间，也不敢多问。完了后大家再一起
去看一遍病人，attending再补充问病人几个问题，做点体检，大部分时间都花在和病
人解释病情和治疗方 |

发帖数: 187 | 11 十月上旬开始的第一个面试，到目前为止面了5个内科program。拿到3个prematch。最
后在3个中选了比较满意的一个把自己卖了。其他的面试已经取消。
回头想一想，大概有几点使我赢得了这些program的青睐。一是credential还不错，另
一是申请的strategy相对正确，再有就是后期包装好，面试准备也比较充分。
面试过后，感触比较深的，想提醒大家一下的有几个方面。其实好多是在老生常谈。前
辈们基本都提及过，觉得有一些是没有得到大家的足够重视，需要再重复重复，有一些
是感觉和很多人的理解不甚一致，需要大家再探讨。
首先想说的是英语：我面试前最担心的就是因英语不好淘汰掉。所以找了个tutor帮着
练习面试。本想在家和LD练，但想想LD和我都半斤八两，也帮不了太大忙，就咬牙花钱
找了个tutor。效果还不错。几次面试中，有两个program的PD夸奖我英语好。真是受宠
若惊。这才猛然发现他们认为的英语好和我们认为的有所不同。我们很多人都以为英语
流利最重要，但看来不是或不完全是这样。在流利之上，他们更看重的是发音和口音。

发帖数: 428 | 12 以前在国内工作时，中午attending都喜欢在休息室打扑克。院里觉得对患者影响不好
，发文不许打。许多科室都执行了，attending就散了。我科主任不置可否，把主任室
让出来，让attending们中午玩（主任室在病区外）。
然后，别的科有突发情况时，值班小大夫和护士往往蒙头转向。我们科从来都是“镇定
自若”，往往是三～四个高龄attending上衣口袋插着扑克，外加一帮小住院在第一时
间冲到病房，患者家属都特别感激。结果是，主任中午在家呼呼睡觉，值班小大夫给
attending们买完午饭后，就开开心心地陪小护士。

是呼吸科。好处是可以熟悉医院环境，坏处是（也不能叫做坏处，因为让我马上开始很
难）不用自己写note，不用自己写order，等于是旁观。所以我也有些着急后面怎么接
手马上开始写病例。
早晨6点起来，6点50到集合会议室，准备参加7点半的morning report，结果当天改成
其他直接上临床的人的orientation。chiefs说我跟的fellow一般8点到，让我8点左右
page她。所以我就在旁边的电脑房间等着。8点我page我跟的fellow。匆忙赶到的时候
，当时他们已经在查房，匆忙和他们握手，跟着去看第一个病人。组的成员是一个on
service的attending，亚洲人，人很好。一个2年级fellow，一个2或者3年级fellow，
一个新fellow，然后就是我。是一个声音嘶哑同时有肺部肿块的病人。这些细节后来发
现必须记住，因为我记不住病人名字，后面都混到一起了。然后第二个病人，是一个肺

今天第一封LOR被upload了。
总结的小体会就是：
1.同一个attending可以写多封LOR的，在ERAS和cover letter里要指定specialty就好
了。为了保险起见，同一个attending的不同信件我让他们分别寄出。给他们提供了
prepaid的fedex信封。
2.一定要提醒letter writer随信要附上cover letter和DSF，我幸亏打电话提醒了，写
信的attending根本就没注意，没有把这两个文件放进去。打电话以后又拆开信封，重
新放进去的。
3.签字一定要非黑色的笔。
4.我的LOR在收到10天就upload了，比传说中的3－4周短，可能是现在还不是高峰期吧。
暂时先想到这么多。

我第一个面试练习是和一位IM attending，他问了一些常规问题，不难。 他说我说的
太多了， 给他的感觉是回答并且提供了太多太多的信息，以致于我在lead the talk.
他建议我先简短回答，然后看看attending的反应如何，再决定是否要展开讲。另外每
一位attending在前30秒都会已经对你有一个几乎成定论的印象，所以前30秒很关键。
我回答strength时总想举例，他说给他的印象是总想证明自己，反而不真实。所以什么
时候说，该说多少，很难把握，还需要多练习一下才行。
第二个面试练习是和一位path attending，他问了几个general Q, 我给了几个general
answer，他说这不好，我的回答必须unique，要结合自身的情况，给attending留下一
些印象。另外谈research时不能太深入，由浅入深，一开始点到即可，根据对方兴趣再

以有较大不同。最近有几位同学对这个问题比较关心，我说说个人体会，算是抛砖引玉
，也给今年面试的同学做个参考。

贫困区的病人往往年轻些，20-50岁居多。因为大多没有保险，重病人多是因为平时得
不到适当的医疗所导致。AIDS/吸毒病人比较多，另外也经常见到Asthma exacerbation

在版上学习到了很多，所以我想把自己的一点点心得也跟大家分享一下。面试是非
常重要的，这是唯一一个和program面对面交流的机会。这个就像相亲一样，听介绍人讲了
对方的条件觉得非常不错，但如果见面的时候感觉不好，肯定不会再考虑交往下去了。
面试的时候看什么呢？不同的科可能不一样，对病理来说，好几个attending都跟
我提到他们对CMG的concern最主要是英语够不够好，将来工作时能不能顺利交流，另外一个
是personality，将来能不能跟同事好好相处。所以英语是重中之重，再强调都不过分
。我自己看talkshow，最喜欢Oprah和barbara walters访谈节目。她们俩说话都非常有
水平，给人的感觉很好。而且这种节目本身就是interview,问的东西很有深度，看两个
人之间的互动可以学到很多东西。面试时不同的attending本身性格也不同，但我觉得
能做到和善，自信应该就不成问题了。面试时，几乎每个program都会问将来的plan是
什么，所以有个职业规划也很重要。你不能说现在还不清楚，这个给人的感觉不好。无论如何说一个，让人觉得

OB中如何树立好的印象和推荐信：
inpatient OB：细节打动人
1.先和medical student介绍自己，做朋友，然后问他们是否介绍一下attending的习惯
和查房的schedule这些background已经常见的提问。
2.和nurse和student熟悉以后，可以了解哪个attending是否喜欢带教学，如果确定后
，问attending是否可以跟他几周，这样写的LoR也strong。
3.眼里要有活：帮助查资料
4.找LoR： 看着attending的眼睛说Could you please help me to write a strong
recommendation letter for application? 如果对方很肯定地答应，这个推荐信应该
不错。如果是再次申请的，用旧的 AAMC number，现在就可以要推荐信了，可以装在信
封里面封闭，拿在自己手里。最好有1个月的时间给attending写信的时间准备。如果医
生要drift template，可以给他。

就是你去找一个case，你present一遍，最好能写下来，或者录下来，然后自己听这个
case，跟什么compare，最好是step3，uw题compare一下，他的题其实是非常好的case
，有用的信息就非常详细，没有用的就非常简略，这就是非常标准的case
presentation，你最开始找不到case的话，可以先看这个题干，看两遍，看完了把电脑

如果你心里担心。可以写电子邮件问 attending是否会给你一个 "strong" letter。

另外，你观察一下attending平时说话的baseline.
我有一个attending, 平时说话就很positive. 我有自知之明知道自己是level B. 但是
因为
他说话baseline就很高, 即使像我这样很普通的人, 他也会给我很多
compliments, 这样的
attending, 写的推荐信，基本不会太差。事实上有很多人问他要推荐信。他跟我说过
去的两个月里写了几十封推荐信。
假如有另外一个attending, 平时说话就很negative, 对自己和对别人要求都很高， 给你
反馈意见的的时候告诉你一堆需要你进步的东东 (这当然是好事， 你可以根据他的意
见改
进自己)。但是也有可能他的推荐信里也会写一堆需要你进步的地方.
你好不容易找到observation 的机会，好不容易能找人帮你写信，没有什么很多的选择

，是可以成为第一作者的。 另外有些医生，经常有很多科研，正好到你的时候，科研

自已医院没有hemOnc，去别的医院做elective，什么都没有也不一定不能match
要会写irb的申请，过多久才能批准，是不是需要consent form。一般有regulatory 的
一个人，他是很清楚的，问问是谁，

自己单独一个人一间办公室，这是其它rotation所没有的。前3天中间还休息了一天，
学校看咱们orientation连续7天，给安排了歇一天。这2天里，就是由senior和
attending教我怎么用系统，怎样用各种模板写病例，碰到各种情况该找谁等等，剪刀
加浆糊轻松地完成了一些follow－up notes，attending还给了一本书，让咱回去读，
他会在我以后的几周里再一起过一遍。
然后第4和5天是周末，除了到ER呆了半个晚上，由senior手把手带着完成了一个
admission，就又休息了。第6和7天senior走了，自己单干了，发现senior演示的东西
我没有全记住，全部自己干和观摩还是不一样，好在attending很approachable，很有
耐心。基本流程就知道了。
第二周开始琢磨怎样将病例写得有板有眼了，前面一周基本上是copy和paste以前的病
例，有时候没有根据情况调整，被attending发现委婉地指出来了，有时候别人的不一

Bowl。
我简单说一下这个活动的规则：
1. Attending们出题，专业不限，简单有趣的case就行，但答案只能一个，必须
evidence based。
2. 由resident组成两个team来抢答，5个ID一个team， 答对50分，答错扣10分， 直到
答对为止。
3. 标准答案由出题attending提供，这个attending具有不可挑战的权威，当然仅仅是
在这个答案上。
活动时间1个小时。到点算总分，获胜队拿大包子。
然后由获胜队提名最佳题目，出题attending拿大包子。
欢迎不参加活动的extern， resident和attending围观，可以自由发言，但主持人不会
予以理睬，板斧们可以酌情给予答对者奖励。
板斧们请帮忙到board申请经费，大包子应该500wb一个，lol |

发帖数: 538 | 31 做 fellow 的时候可以在本院独立值班挣点小钱。有一天一个36周左右的病人围产期心
衰入院了。入院后诊断成立。开始治疗。一顿利尿，症状基本没了。Division最二百五
的attending consult后拟定计划37 周引产。我当时知道有这么个病人，白天还和
attending一起看过。心里就有点疑问。为啥非等到37周。现在状态好，生了不就得了
。就祷告了一下千万别在我值班的时候生。
病人高龄，多年前生过一个。方案就是早上epidural，深麻，减少疼痛刺激。不让病人
push，等到孩子降下来产钳或vacuum助产。总之了，就是尽量不增加心脏负荷。结果
引来引去，到我值班那晚上要生了。这一看就明白了，祷告真起作用啊！交班的时候
division头问，forceps有信心没。当时的回答是应该没问题。
具体过程有点忘了，就是不停的想别出事别出事。。。。。结果孩子心跳开始下降，赶
紧去看血压有点低，宫口开全了，头还挺高，就请麻醉师帮忙升下血压。麻醉是很深的
。效果极佳。没过几分钟，chief又打电话说不行了，心跳不停有问题。拉到手术室做

Obama, a site that has now gone private (which is revealing in and of itself
). It was chock filled with one religious leftist post after another

新的 DS-160 表格取代了以前的 DS-156、DS-157 和 DS-158 等表格，只能在网上填
写。
DS-160 适用于除 K-1/2 (fiancee) 以及 E-1/2 (Treaty Trader) 的所有非移民签证
的申请。
在3月1日之前，申请人可以提交 DS-160 表格，也可以仍使用老的 DS-156 表格。找
不到 DS-157 和 DS-158 表格的，可以去参考本人的博文。
除了在线填表，非移民签证的申请还包括预约签证（购买电话卡）、交纳申请费和签证
