在美国没有主治医师，副主任医师和主任医师一说。在美国具有独立行医的医生（住院
医生和专科训练毕业后，可以独立签发报告，独立手术等等）叫Attending physician
，在大学这些人都是faculty，大学里面Attending是走教授系列，所以Attending的职称可以
是instructor，Assistant professor, associate professor,或者full professor。
所以把attending 翻译成主治医生不确切，跟国内的主治医生是不同的概念。国内的主
治医生是一个职称，介于住院医生和副主任医生之间。Attending在美国不是职称。
在美国非大学医院（不走instructor，Assistant professor, associate professor,
或者full professor的系列的地方），那五花八门了，反正就是没有统一的规定，有些
地方叫associate staff physician，staff physician等等。这些职称跟大学医院的
professorg系列是无法对应的。 |

发帖数: 118 | 16 一个月的cardiology轮转就要结束了，这是我第一月在病房，跟大家分享一下。首先是
回报病史，刚开始真的是很烂，好在我的attending和fellow都很好，经常提醒我如何
抓重点，病史的第一句话很重要，only cardiac hx 不要讲不相关的，这样才会引起
attending的注意，结束HPI后要详细地叙述过去的cath, echo,thallium study.然后再
开始pmh..,medication的时候先讲和心脏相关的药物，一些眼药，外用药没有必要汇报
。PE的时候I&O,Tele 毕不可少，lab 先汇报异常的，其他的一句话带过就可以了，现
在的EKG上都有计算机打出的结果，汇报前好好看看就行了，最好有个圆规，我是管护
士要了一个。A&P很重要，用我们attending话讲，you know where you are and where
you are going.汇报时一定要自信，即使错了也没事，不要用询问的口吻，而是要说
服对方。这里都是EBM，所以常用的药就那么几种，再就是摸准attending喜欢的药。比

打电话接电话都能感觉到级别摆在那里，要是PGY比我低的打电话来，多半都很谦虚，
fellow打电话来都牛哄哄的。有一次一个GI fellow 要 cryoprecipitate 我没给他，
他马上态度极为恶劣，吵了几句就说 give me you attending's number，我第二天跟
attending 说这事，attending 说他对我很谦虚啊，还跟我说要跟你解释一下他态度不
好的事情呢，kao！
我觉得你这事，第一对方那头病人不急，第二你这边自己主治医搞定了，你办得很漂亮。
病理这边有的attending的态度就是，我们是为临床医生服务，不论他们要什么，都给

我和我的attending 被一黑人告 discrimination,我和我的attending绞尽脑汁也想不
起来那黑人长啥样，是什么时候发生的，besides 我的attending是个白人老头，对黑
人非常和蔼，谁discrimination也轮不到他。我呢，暂且不论对某些少数族裔如何评判

发帖数: 810 | 19 一周的病房OB终于结束了，每天都郁闷无比的回家。这家医院地处downtown,非常之繁
忙的说。除了周一早上8am在办公室morning report之外，其余几天就是早上9am
attending 和residency在病房汇合，一边rounds 一边report,每看一个病人后
attending就提出自己的意见，residency再补充一下，几乎没有太多讨论时间，更别说
提问了。然后attending就当场写patient notes,residency就在电脑上开检查和处方，
同时立刻打电话给需要会诊的部门等等，这样一个病人忙完了就接着去看下一个。而且
病人分布在不同的楼层，真的是走路都带小跑。查完了基本都12点了。residency再一
路小跑的去买午餐，带着午餐参加中午的conference.下午attending就回自己的办公室
处理日常事物，所以我也没人带了，基本是参加完午餐会后我就走人了。
不知道是因为神内本身就没有intern，大家也没有带人的习惯，还是我因为语言原因很

今天组成员发生变化。attending, 新fellow, 一个第二年，一个学生，我。
今天还是round老病人，下午一个new admission, 新fellow带着我们去看的，fellow
present case, attending 教学并说了处理意见。没有会诊。
问了fellow, 所有呼吸科病人都有自己的intern, resident执行所有医嘱，写note. 有
部分attending是hospitalist, 有部分是呼吸科的specialist.
New admission 新fellow自己要看和present。会诊会让我们去。第二年的resident很
积极要做new admission, fellow 说必须她做，没办法是规定。
下午三点四十就让我们回家了。想到麦地前辈的贴子，心里着急啊。死乞白赖跟着
fellow要求看她如何做dicTation，能学一点是一点。因为notes已经写好，fellow也就是
念一遍。四点二十做好了，再次确认在呼吸科选修不可能自己管病人，由分配的intern

刘大夫曾经开医板非诚勿扰，曾经给coffee许多建议。所以是特约嘉宾。刘现在是PGY3，马上就attending了。
1 出国：04年出国，02毕业，2012 毕业整10年。听说过5年是大槛，没有弄成，所以赶上了10年的。
MDtechnicianlab manager match
10 step1, 11 step2ck, 11 隔3个月CS。 从2步才知道麦迪，后来就容易了。
悲惨故事：9/3 submit只6个面试，9/1 submit 只3个面试，所以9/1 就交了。
一步98分，CK99, CS一次过。9/6，第一个IV，内科。告诉麦地后，潜水大哥给了私信，告诉了他们有什么题。后来，再报的面试，都有人私信来帮助她，所以很受益。
除非特别低调的人，最好还是报面试，面试拿到了，不会跑的，哈哈。。看的会有前辈，他们还是愿意帮助的。
9月IV高峰，90%IV都是在9，10月收到的。有朋友10月底才申请，这时候IV已经很少了。这时候，大概也就会拿到3，4个，前面的就都错过了。

1. 分数很重要，250分数要比240拿很多（至少是前期）；
2. 如果分数已经确定，在等IV，可以打电话（口语好），发电子邮件骚扰争取IV;打电
话时，准备好自己的相关信息；
3. 一定要争取clinical experience, 如果不做，拿IV大受影响；
4. 如何拿externership，做好心理准备会碰很多壁，不灰心，创造机会；
5. 发挥中国人优点，尊重代教老师，training结束时，一定要感谢医生以及护士，搞
好关系；
6. Externership，大医院需要很多paperwork，私人诊所不需要；Externership要在推
荐信里体现出来，说明你有hand-on；
7. 做externership和OB时，不要给老师增加麻烦；做人最重要，让对方comfortable；

今天很机会和一个attending聊了一下，我特地问了一下，how to behave well in
observationships.下面是一些我还记得的，有些还是满有意思的。
1． 提问题不要占用时间，在走路的时候提，不要在看病人的时候. If somebody is
in a hurry or late, do not ask questions. Say: I am really interested in xxx
and have some questions here. I am thinking maybe we could talk about that
later.
2． Be OUTGOING. Do not be shy. Although they are always in a hurry, they

我总是受Senior嘲笑是有道理的。 我的第一天，第二个病人是个年轻的女性，据说曾
经是运动员，脊椎受伤史，预约了手术，长期服用镇痛剂，否则没法正常抱孩子和生活
。当时碰到她，她就象SP那样的模范，让我惊讶嘀咕，也让我同情。 然而在开镇痛剂
的时候，Attending A表示他以前看过她，镇痛剂上瘾，明确表示不给高效的镇痛药。
我真是难以置信，怀着一线希望，让她直接和Attending A 谈，看她敢不敢为自己的病
情争取，还是真的只是为了高效的镇痛药而已。 我欣然看见她还是面对了Attending A
。 然而被他语重心长的教育了一通，她健步如飞的溜了。我哀怨的看着她的背影，心
碎了一地。Tell me sweet little lies. 我也应该知道会有这种情况，但还是没有准
备好。
后面自然还有别的要镇痛剂的，我心里有障碍，躲在Senior背后，让他们去处理疼痛的病

门诊某西裔病人，英语不好，需女儿翻译。自述上次看病后血压仍持续高，所以自行把
某常用降压药的计量，从一次5mg一天一次加到一次5mg一天两次。正好病人带了药瓶，
我通过女儿翻译，确定了该药为10mg每片，每次吃半片。于是报告attending，病人自
行加了药量。不料，再跟attending进去一起看时，病人自己用不流利的英语跟
attending说，不说一次半片了，楞说成了一次吃一半剂量，attending问，那就是一次
2.5mg一天两次啦？母女俩竟然也表示同意。吓得我赶紧喊wait a moment，叫病人再把
药瓶拿出来，把10mg一片指给attending看，再拿着瓶问病人一次吃半片还是四分之一
片，病人总算才知道自己说错了。要不是有药瓶，我差点就要窦娥了。 |

生，我敢说3个月内可以让他做心脏冠脉造影。这就是为什么在CathLab，熟练PA的作用
比第一年的Fellow还大。我们当时有个Cathlab，一般的心脏支架是PA带着Fellow做，
Attending坐观察室。
NP/PA接管Procedure不会防碍Attending的收入，NP/PA受Attending的Supervise，
Billing在Attending的名下，何乐而不为呢？
但是，雇佣NP/PA是要化钱的，这也是为什么有Procedure，Attending们都自己去做。 |

欲动是何其明显，突尼斯已经基本成功，老大哥埃及也顶不住了，在中东的沙特好像也
有异动的报道。
突尼斯，埃及，沙特这些国家有一个共同点就是经济相对稳定，发展势头也不错，民众
相对富裕，整个社会已经稳定发展了几十年了，但是政治就比较保守，最明显的就是国
家元首基本可以连续执政二十年以上。不过街头革命的意义有多大，成功率如何，对国
家政治进程的帮助有多大，还值得期待。这一波浪潮是否能够轻易化解，穆巴拉克，阿
普杜拉等等估计得废一番心思了。
二十一世纪的第二个十年有多少期待？

Default Delta Flight Attendant Wouldn't Help
My daughter was flying today from Chicago to Dulles in Virginia. Her Delta
flight was delayed and caused her to only have 30 mins to get to the next
flight. She knew she might have issues and alerted the flight attendant and
asked her if she could get off first or be one of the first to get off so
she wouldn't miss her flight. SHE SAID, "NO." I cannot believe it.
