Each year after match day, the Student/Resident/Fellow Committee of ACAP
will host the "Welcome and Career Development Seminar". It will be a good
time for us to know and network with each other, and learn from our senior

发帖数: 125 | 3 有同学发信问我externship做操作的问题。我就简单说一下。
关于操作的话，主要看带你的attending。其实，现在IM的physician很少做操作的了，
因为有介入，一般都是他们来做。我那次是非常幸运，刚好值夜班，之前也有跟我带我
的IM attending沟通关于操作的问题，他说他自己很喜欢，但是因为收入少和费时间，
他一般都不做。我们值夜班的时候，刚好那个病人要做chest tube，我的attending就
跟ICU 那个attending沟通，让我打下手，谁知那个ICU的attending人非常好，也估计
是我帮忙翻译的缘故（那是个中国病人，不会说英文），就让我几乎全程做了这个操作。
所以，如果是hands-on的externship，我们是有mal-practice insurance cover的，如
果attending有操作，肯放手，我觉得我们都是可以做的。但是，这种机会非常难得。
另外一次病人要气管拔管，带我的attending想让我做，不过后来病人拒绝了。
所以，在做rotation的时候，最重要的是和你的attending打好关系，做事努力，把握
机会！ |

NYHQ做OB还有一些问题，所以希望写下自己的经历，对后来人有所帮助。
首先非常感谢ACAP的帮助，让我们在NYHQ能一直有机会。我来到NYHQ医院做了一个
月的OB. 虽然知道这个机会来之不易而且还有很多XDJM在报名排队，我在来之前还是有
些犹豫。因为可以在我所在的城市里的教学医院做OB，为什么要离开家人和工作，来到
一个社区医院做呢？其实来到第一天的见闻就打消了我所有的顾虑，一个月确实挺值的
，只是没有机会再做长一些时间。
NYHQ位于法拉盛区（Flushing），医院大厅标注着英文、中文、韩文、俄文等等，
足可以看出医院的多元化（diversity）。第一天先参加orientation和infectious
control training， 然后去街对面GME办公室递交application form等paperwork，再
回到医院办理ID卡，到五楼找chief resident报道。一批observer一般有四人， 两周

发帖数: 704 | 6 在迷失混乱中，度过了自己intern的第一周。我一开始被安排在肺科轮转2周。比起其
他interns, 我一周都好像在游手好闲，有点不好意思，暂且也跟着大家写一写感想。
紧张的intern, 帅气的senior，方向迷糊的fellow.
虽然有两周的上岗培训，由于分到的是另外的一个医院，第一天开始的时候，又有一个
简短的培训。对我这种没有经验的人来说，医院的房间编码是很让人糊涂的。 我一开
始跑到一个培训的地点，发现周围的intern男生的穿着和头发的亮度有点可疑，比内科
好像要正式的多，才发现我搞错房间了。
后来发现还有另外一个PGY3 resident和我一起到肺科。他看起来是个聪明，可爱又帅
气的医生，大家很爱和他开玩笑。他刚刚结束ICU的轮转，对我说，肺科的轮转很轻松
，如果有什么事情，就怪到他头上好了。后来我才明白他说很轻松的意思。 Attending
的重点是带新来的fellow。根本不在乎我们这两个residents来不来. 我自己也明白

一直在临床混，到一个科喜欢一个科。对年初来PD反复提醒大家申请Fellowship的通知
熟视无睹。混到六月底，发现平日平易近人,和竭可亲的PD见面沉默或视而不见了，觉
得问题有些严重了，赶快表忠心也要申请Fellowship. 看见PD终于笑了，我一高兴，
问, 同时申请两个科行不行？（那就是PD要分别写两封推荐信）。PD愣了一下，说当然可
以。我更兴奋起来，那申请三个？ 四个？ PD一时无语。。。那你花很多很多钱的！
骑虎难下，注册交钱，7月初独立曰终于掉下来一个长假期，突击写PS，填表准备申请
。给以前的老师们要推荐信，还加上一句，如果你们度假，可能来不及了，不写也可以
（我心想，那不就省心了？！）然而，工作狂的老师们居然大部分在7月中旬或八月之
前把推荐信上载了。以为就是自己草率而已, 不久就欣慰的发现以前几个犹豫不决的三
年级生也已急忙按时交上了fellow申请。
和住院医申请不一样，Fellowship申请网上邮局一开通，第二天开始，邀请函就像不要

extracurricular activity。2月中旬在Houston完成CS。考完還在休斯頓豐田中心邂逅
了火箭隊的教頭Kevin McHale.
在过去两年之中，完成了U Toronto, Baylor, Mount Sinai, Yale之后, 這次来到

发帖数: 5 | 23 USMLE 一路走来，从版上学了很多东西，虽然碰巧赶上了个2008 opening, 但毕竟大部分考友都是match in 2009, 随便灌水，祝你们好运。
面试人人准备，可几句下来，pd 或是其他人就会有了impression. 近了这个program 一个多月了， 我觉得我的PS并没有突出我的strength. 如果你有同样的感觉， 面试一定要emphasize.
给了10个左右的tour, 1 CMG, grad in 90ish, 1ABC 4th yr student, 1 American 4th yr student, 其他为indian为主的亚洲FMG, grad in 2004-2007. 我们的weakness就是毕业年限长, 长的还不是一点半点：）我们的potential毋庸置疑，但pd挑的人是来了就能干活，所以尽可能表现的对现有inpatient care 熟门熟道。突出observship. 我们的优势：在美国呆了较久，比较熟悉culture；我们年龄偏大，较成熟。印度同学们来势凶猛，Philippine,Napal，Thai紧随其后，多是２００７，２００６... 阅读全帖 |

，今天来聊聊第一年的intern生活。其实正经开始第一年之前，从来都没有做过
inpatient的ob，体制不熟，英文吃力，不少medical term都表达不出来，记得第一个
rotation是在心内，病人straightforward，每天只有两个admission，而我却天天都是
8点以后到家，早上6点就去医院preround病人，即便如此许多东西还是似懂非懂，完全
由两个超好的resident照着，我的感觉是自己与这个环境真是格格不入，很像初到七连
的许三多，背后也流过不少泪，想方设法提高自己，终于半年后拿到了一次最佳表现奖
，有带我的住院医感叹“you are so efficient”，还有的评论：she was the first
intern who showed team leader commitment，工作非常得心应手，现在越来越多的在
享受医学带给我的乐趣，和生命带给我的思考。既然来写帖子，在这里毫不保留的与大

虽然美国住院医训练制度相对标准化，但是各个医院具体情况不同，训练的侧重点也可
以有较大不同。最近有几位同学对这个问题比较关心，我说说个人体会，算是抛砖引玉
，也给今年面试的同学做个参考。
1. Patient population
贫困区的病人往往年轻些，20-50岁居多。因为大多没有保险，重病人多是因为平时得
不到适当的医疗所导致。AIDS/吸毒病人比较多，另外也经常见到Asthma exacerbation
, Sickle cell crisis, 严重感染等。Resident的自主性比较强，procedure比较hands
on一些。气管插管，腰穿等等很快就certify了。不过抽血等scutwork往往也偏多。这
里的病人往往不太按时随诊，相应而讲纵向观察病情演变的机会略少。
富人区的医院病人往往年龄偏大，70-80很常见。病人除了入院的主要病情, 既往史往

离毕业还有110天。
最近在Boston待了约五个月，圣诞回了趟上海，新年开始就一直在Boston复习CS和一些
extracurricular activity。2月中旬在Houston完成CS。考完還在休斯頓豐田中心邂逅
了火箭隊的教頭Kevin McHale.
在过去两年之中，完成了U Toronto, Baylor, Mount Sinai, Yale之后, 這次来到
Harvard Med。
2013年10月，同样是哈佛的附属医院，原来要去位于剑桥的Mount Auburn的ER急诊实习
，结果那边最近不接收学生，就去了Dana Farber/Brigham and Women’s Cancer

ERAS CV 很重要，避免dilution，要找人把关
我们在申请的时候，每个program拿到的申请会非常多，大家都会很快的go through你
的材料，如果你有很好的闪光点，一定不要把相关性不大的和你的闪光点堆到一块，由
于你cv太多不相关的东西，你的闪光点被忽略。
比如，你在国内有一年病理工作经历，这是非常好的，放在前面。什么一等奖学金啊，
是不值一提的东西，相比之下。会觉得你做事 主次不分。
申请病理的时候，如何写你的research 和publication？ 在ERAS里
除了一些非常好的大学，注重research，你的background和他们match，有一些帮助，
举例我朋友拿到R21 grant，match很不错的program，有这样background的不多，你的
research和他们match的很少，大部分中小型的university，pathology program，对
research不感兴趣，你的reserach 可能会给你减分，所以你在ERAS cv上展现多少，一

fellowship的faculty）
工作方面：

每个病人至少预留20分钟，这是比较保守的时间；但即使最快的也要10min，别忘了还
有在不同病房穿梭的时间；
一定要亲自问病人情况，因为护士可能情况不报给PGY-1只报给PGY-2，而恰好有些刚上
任的PGY-2喜欢show-off自己的领导能力。
2.关于Intern的职责和如何表现：
Intern的两大工作：1）采集信息；2）干活（跟各种人communicate + put in order）。

你，什么地方有工作。推荐~~~~~~ 大多数工作是靠推荐人推的，要和attending搞好关
系。Attending介绍，是最牢靠的招人的方法。在住院医的时候，要有特别帮你推荐工
作的attending。拿到好推荐信。
找工作 number1 要素，不是你的knowledge，第一是teamwork，第二personality，第
三是你的knowledge。你能不能跟人搞好关系最重要。 一定要有advocate！ 有10个
attending都关系不错，不如和一两个有力挺的attending关系很好。
22） 为什么美国住院医不热衷申请MD Anderson和 Sloan Cancer Center？
如果你在大program出来的，没必要要去。
如果你program非常小，可以考虑去， 特别是programs 没有 癌症中心的。
因为这两个programs需要cutting。很少的综合医院的fellowship要cutting。像我当年

1. If you get into both Harvard and UCLA, it doesn't make any difference
which one you attend. Whatever little you lose by giving up Harvard, you
gain by having a higher class rank at UCLA (this is in the paper).
2. The return to investment was around 17% for students who went to
elite colleges in the 1970s. It's much lower now with rising tuition

certainly left their mark this year, as students prepare to attend the
nations' top universities next fall. Among the 409 students in this years'
