to do it, do it under a physician's close monitoring.
TKIs cause interstitial pneumonitis (this is a very serious condition,
prognosis worse than non-small cell lung cancer) in some patients. in Japan,
about one in every 20 patients on TKI develop this in a relative short time
of period but they are under close monitoring so are detected early and a

classified into different risk group, low and high. The goal is to prevent
thromboembolic event, using aspirin or hydroxyurea. About 50% patients with
ET have JAK2 mutation. A oral JAK inhibitor ruxolitinib was recently
approved by FDA to treat myelofibrosis (end stage of ET). Several clinical
study is on going to test the drug in ET before progressing to MF with
promising result. |

specifics of the treatment depend on the type, location and the patient.
there are a lot of trials using pathway targeted therapy, but most are
proved to have little to no efficacy at all.
David Gutmann (Wash U.) is one of the biggest researchers on this. but I don
't think he has better treatment for these patients. |

There are azathioprine, 6-MP, TNF inhibitors here. You can ask the doctors
in China weather they have these medications. These medications need close

我也部分同意楼主，一些恶霸提出的概念确实有问题，导致不少人送死。
楼主列出的这几个领域，我不是全部了解，所以只说说我知道的。
我知道的好像p53的inhibitor在肿瘤方面没有什么作用。但是有一点很奇怪，在很多肿
瘤细胞里面,p53的突变率很高，而且小鼠模型里面，p53敲除确实可以导致各种肿瘤。
只能说肿瘤发生发展太复杂了。，不是一两个基因能说明问题的。
关于肿瘤干细胞，sean morrison在一篇nature文章中已经证明在某些tumor中(黑色素
瘤)，肿瘤干细胞是不存在的。
关于个性化治疗，我觉得这个还是很有发展前途的。最近有几篇高档次的文章表明在
myeloid leukemia病人中Dnmt3a中，有一个点突变频发，并且小鼠模型证明了这一点，
这样可能就找到了一个可能的myeloid leukemia致病因素，在对病人诊断筛选的时候，
这个突变可以作为一个指标来分类病人，这个应该就是个性化治疗。至于下一步的药物
选择，这就需要结构生物学的帮助了，看看时候这个突变引起Dnmt3a结构的变化，是否
能够找到靶点等等。

Inhibitor是从信号传导角度来抑制癌症，理论上没问题，实际要看数据。 大药厂还是
小药厂不重要。 小药厂发现好东西可以卖给大药厂。 Genentech自己搞出的药并不多
，我在做博后时发现的一个抗前列腺癌的特异性抗体，Genentech拿去做动物实验了。
