HPI, esp the sequences of the symptoms are very important in neuro patient.
If the patient has CVA in the past, you need to know the patient's function
status, the left over symptoms from last stroke. Are there any tests
doppler, MRI/MRA have been done in the past. Does the patient on any
preventive meds for recurrent stroke or not.

1 考试：东西理解，要熟，没捷径。具各人情况，看待别人的经验。目标不同，成绩要
求不同。至少要有一门考得非常好。
什么时候考试，要看准备take 多大risk。时间短，波动性大，分数不稳定。
2 申请：7/1 开始，可以上交到ERAS，之前应完成推荐信，OB之类的。
（1）做OB可以增加interview数目，同时会增加interview的信心。
抓住机会做OB或ext.，同学，同事，朋友，买。可增加对美国临床的熟悉。要表现的谦
虚，肯学，勤快，不多事。找好的attending.
（2）推荐信，最好有想申请科的医生的，实在没有也可以。推荐信要好，没有好话，
不能用，就是牛人的，如果没有好话，一定不能用。实在没有的话，找中国人写的也行
，但一定要确定是好话。
推荐信重点 （由重到轻） 1 好话 2 牛 3 最好在美国的医生的，没有的同学写的也
行，但是话要好 总之，谁的话好用谁的，不论这个人来自哪，干什么，只要是好话，
就首选。争取可以看到推荐信的内容。遵守eras规定，但是要灵活掌握，一定要知道是
非常强的推荐。

别挑来挑去了。
2。内科的不管从哪里毕业，工作机会是大把大把的。小program人少有时候更象家庭一
样。不过千万别抱着去了哪里都会有很多朋友的想法。program里每个人都unique,怎么
去处，靠你自己啦。
3。有时候看看abd ct report如果有肝脾肿大。echo有瓣膜病变的，X-ray 有
pulmonary infiltration or consolidation的，再回去在病人身上好好找找去。 |

举个例子：case 12那个forgetfulness包括eye exam, neck exam, CV exam,
pulmonary exam, abdominal exam, neurologic exam，你们是怎么在2分钟内完成的？
2.你们体检的时候是光着手，还是带手套。如果是光手，是不是也直接去摸病人的脚？
如果戴手套，那么还洗手吗？
3.FA上面说你如果没做某个体检，就不要写你做了，这怎么可能？2分钟不可能做什么
体检，那岂不是没得写了？你们写不写？

疾病。 控制高血压，高血脂，吸烟这些危险因素重要啊。这里的医生/医学生并不知道
具体的病情，什么样的症状和遗留的功能损害，所以最多只是提供一些建议。
肺炎是中风病人的常见并发症之一。 60% 是由于中风导致的吞咽功能失调，包括咽喉
肌肉的运动和感觉功能，或其他原因等等而导致的吸入性肺炎。 可能是吸入口咽的细
菌，也可能不是细菌，是化学物比如胃酸对肺部的刺激等。 如果是细菌感染，胸部X光
片应该可以看出。留深部痰做多次药敏选择抗生素应该能够找到合适的药物，如果是吸
入性肺炎，抗菌素是一线的clindamycin克林霉素，或ampicillin-sulbactam氨苄西林-
舒巴坦。头孢，环丙沙星等其他的抗菌素不是很推荐的。如果是化学性的或者气道阻塞
导致的肺炎，抗菌素是不管用的，只是支持肺部的呼吸功能的治疗。所以，根据病人的
具体情况，看他到底是那种原因导致的肺炎，好治疗和预防。
的确如A医生说的，增加血液的粘稠度，比如脱水，蛋白过多，没有抗凝治疗，还会导

算是不定期的感想。打算系列写下去。有钱的捧个钱场，没钱的捧个人场。
1。 真的是勾通太重要了。
我说一遍ORDERES，需要INTERN去做的事情，要记下来，要逐一落实，落实有困难的，
一定要及时勾通，我才能发现问题出在哪里。UPPER LEVEL RESIDENT 是要查一下结果
的， 如果老是对不上，就会质疑INTERN的能力，这样印象会非常不好。比如我说， 要
ORDER LANTUS 10 UNITS DAILY， 我的INTERN记在小本上，说一定落实，第二天一查，
根本没做，说不记得我说过，我说， 查你的NOTE，赫赫写在上面，然后道歉。
PULMONARY CONSULT 写了一堆医嘱建议， 我要求INTERN逐条落实， 因为医嘱多，我三
次问他，是否逐条落实了。 最后回家查他的ORDER， 发现少了两个医嘱。
因为带2个INTERN，2个MS，一堆病人，希望一遍过。所以对于这样的简单之极地事情还
出这样的纰漏。真的是非常恼火。

就是apply的过程，比如genetic的题目。以不变应万变。
对于不会的题目，不要粘题，没有思路，跳过去，回头再想。有时是瞬时block out一
叶障目，后头再看的时候会发现豁然开朗。不值得浪费太多时间在一道题目上面。
关于排除法的应用，首先排除自己熟悉的选项，再排除自己不熟悉的选项，如果自己熟
悉的选项不能排除，怎么能排除自己根本看不懂的选项。解决方案：如果最后自己熟悉
的选项不能排除，还是选择自己熟悉的选项。因为太超范围，太偏太远的东西考的可能
性毕竟比较小，较大点的可能是可能是自己熟悉的，但是自己没有做到很好的apply或

good by to USMLE
Material: MTB3 and UptoDate. Step 1/Step2: 250/249 but those were 3 years
ago. Step 3: 227. Time: on/off for 3 months.
Good luck to everybody.
Not a whole lot of thoughts about this test. if you can memorize MTB3 and UW
, then there should be NO problem to pass it.

NYHQ做OB还有一些问题，所以希望写下自己的经历，对后来人有所帮助。
首先非常感谢ACAP的帮助，让我们在NYHQ能一直有机会。我来到NYHQ医院做了一个
月的OB. 虽然知道这个机会来之不易而且还有很多XDJM在报名排队，我在来之前还是有
些犹豫。因为可以在我所在的城市里的教学医院做OB，为什么要离开家人和工作，来到
一个社区医院做呢？其实来到第一天的见闻就打消了我所有的顾虑，一个月确实挺值的
，只是没有机会再做长一些时间。
NYHQ位于法拉盛区（Flushing），医院大厅标注着英文、中文、韩文、俄文等等，
足可以看出医院的多元化（diversity）。第一天先参加orientation和infectious
control training， 然后去街对面GME办公室递交application form等paperwork，再
回到医院办理ID卡，到五楼找chief resident报道。一批observer一般有四人， 两周
