我05年本科毕业于国内一所三流医学院,然后留校读了硕士,后又来加拿大读硕士,一直
做神经类的研究,对Alzheimer's,Parkinson's disease和central metabolism
regulation以及neuronal development比较熟悉.考虑到加拿大的恶心政策(鼓励其它国
家的医生移民来加拿大,又不承认他们的医学教育,即使拿到了医师执照,工作机会也只
给英联邦国家的医学生),我又有SCI一作二作三作的文章,同时可以找到Canadian
stroke network的director和Canada research chair in Parkinson disease给写推荐
信.因此打算重读一遍医学院,同时想多积累些人脉(我有比较好的分子细胞和动物实验
基础,去一些MD的实验室参与课题很受欢迎).最终的目标是去美国做个神经科医生,不知
道神经科医生在美国专科医生中处于什么样的地位 |

These non-motor symptoms can occur several years or even a decade prior to
motor symptoms in Parkinson's disease.

是有个住院医出了个馊招：给他安慰剂来治疗他的精神障碍。
我们最终没有这么做， 因为所有的治疗应该是informed， 病人必须被告知，否则不道
德。
其实即使给安慰剂（sugar pill， vitamin）病人病情好转也不见得说明什么问题，
医学研究证明：安慰剂就是有效的。
如果一个医生对安慰剂的效果有充分的认识， 他/她就可以成为一个把医学从科学上升
到艺术高度的好医生。
治疗一个疾病是一个team work， 这个团队包括医生， 病人和他们的家人。 在一个有
evidence based 的科学诊断做出以后， 患者以及家属对医生的信赖由此就建立了。
医生实施治疗前， 会认真详细的解释治疗方案， 同时给予病人信心。

这个问题在我们平时practice非常常见，Reglan是一些医生的常用药，对某些
intractable nausea非常有效。 这个病人显然是出现了Parkinsonism，NPH可以是DDx,
但这个病人应该首先考虑reglan引起的parkinsonism。
PCP的诊断是对的，但他的错误是停药。 突然停药可以引起或加重tardive dyskinesia
（TD). TD可以是irreversible，而且不可治愈。 TD是practice中经常引起法律纠纷的
一种disability，所以应该尽量避免。
正确做法是taper off。 我们有很多精神科医生，像benpu，stardust，etc， 你们应
该是处理neuroleptics副作用的专家，这里讲的原则同样适用于所有neuroleptics 引

不能接受的，但针对某个疑难病例的讨论应该符合到版上来的医生的利益，所以我觉得
可以开展一下这样的活动，需要强调的是，所有病例都会被修改得面目全非，所以即使
病人或家属认为这个讨论有他们的影子，讨论内容只能用来学习和借鉴，不能用来照搬
， 否则后果自负。
我建议网友有疑难病例请先寄给版务，由他们修改后发表。
我先开个头，下面的病例已经修改：
【简单病历】
性别：女 年龄：48 1年前发现眼睛睁不开，在某医学院附属医院怀疑梅氏综合症（
Meige's syndrome）现面部、颈部、嘴部均不自主痉挛抽动，放松平躺时症状减轻，每
次抽动时，颈部大筋变硬，类似于人哭泣到极致时的啜泣，吃饭收到影响，容易在咀嚼
食物时突然不自主吸气。曾于北京多家医院多次注射Botox，均无显著效果，现服用药
物为：盐酸苯海索(Artane)每日一片、氟哌啶醇片（Haloperidol）每日一片。因该类
药品对记忆力有可能产生不了影响，曾试图停药，但一旦停药抽搐的就更加厉害。

么大病，因为她有腿麻，所以做了EMG/ncv，发现轻度的neuropathy，这不大可能是行

官方的口径里并没有说到结石姐要退赛，只是无法参加第八期录制。
关于自己的病，结石姐在一次采访中，自己有说过。
在她九岁的时候年，她就被诊断出患有预激综合征（Wolff-Parkinson-White syndrome）。
那是一种先天性的心脏病。她爸爸也患有这个病。
乐观的结石姐把这个病视为她跟她爸爸的一种特殊连接——“因为我们都带着这种病生活，并牢牢记住，它是我们的一部分。”
结石姐说：“人们容易把脆弱当成了懦弱。其实展示脆弱，就是展示真实。你要明白，你是要克服它，而不是沉浸在情绪中无法自拔。这就是我从父母那受到的启发。”
所以，在2015年时，结石姐病发，取消了巡演，她也是积极恢复。
由于不能过于激动，医生建议她少飙高音。
这对一个声音横跨3个八度的人士蛮灾难性的打击。
但是，结石姐并没有理直气壮地“丧”着。

物《新英格兰杂志》（NEJM）上（NEJM是目前世界医学界最权威的学术刊物之一，也是

文章的第一作者，俄勒冈研究院李福中（Fuzhong Li，音译）博士表示，“这些成果具
