Negative,不需radiation。
biospy时左乳发现的两个tumor:
乳头处的tumor HER2是negative (IHC score 2+,FISH negative) ;
12点的tumor HER2是positive (IHC score 3+)。
手术病理显示有两块 specimen, 因为有re-excision。
但是只发现一个 tumor(乳头处),1.5cm, HER2结果和biopsy时乳头处的tumor一样,
IHC score 2+, FISH negative,另一个(12点)不仅tumor没找到,biopsy时放到那的
clip也找不到。后来作过X-RAY, US, MRI,没有发现体内有什么残留。
本来医生说chemo会采用 TCH*6 with HERCEPTIN for 1 year based on the fact that

正进program是从7月2号开始。前两个月都算orientation,grossing有chief手把手教的
,一天gross大标本,第二天signout。第一个星期不知道怎么搞的,specimen不多,中
间独立日还放了一天,开会,看autopsy,还没什么感觉就过去了。 第二周开始标本多
了,chief稍微一放手,我就犯了错误。一次是dictate size,结果没dictate进去,
attending问我也不记不清了,最后用slide上拼着估计了一下,下次要把dictate的号
码记下来这样还可以回过去查,另外一次submit lymph nodes,slide key 写成A1
lymph nodes-4,attending搞不清是4个完整的,还是一个切成4块,所以正确的说法是
four lymph nodes-4。还有thyroid tumor,4cm是staging的cutoff值,得注意。

tumor size, so 4 cm or 6 cm will not make a big difference. However in
other organ systems, like breast, lung, size determins stage. 2.9 cm and 3
cm in a lung tumor will change its stage. Check CAP Cancer protocol for
detail.
Every time you gross a specimen, always keep in mind: orientation, ink,
measurement, weight.....

PA大部分任职于hospitals,这一部分也是薪水相对最高最稳定福利也最好的,朝九晚
五,因为overtime的情况一般都有pathologist residents处理。还有一部分在
reference labs和各种private labs,这些lab因为处理的specimen数量庞大,所以可
能需要night shift,平均薪水比医院略低。还有很少的一部分在forensic和政府部门。
统计来说,New grad PA的薪水大约从75k-90k不等,就我知道的case来说,几个前两届
的毕业生大约都拿到80-85k的offer。Experienced PA能拿到6位数。PA的薪水大约是加
州最高,这些年一直PA工作机会很多,一些有PA program的states(供大于求)和相对
比较偏远的state(ex. Maine)薪水较低。

PA program一般都比较competitive,每个program每年招收的人数很少,小的program
大约5-8人,中等的12-16人,最大的program应该是quinnipiac university每年招24-
26人。我所知道的录取比例大约1/9-1/10。
1.Prerequisite courses
每个program的要求都不太一样,不过大概包括chemistry,organic chemistry,
biochemistry,microbiology,biology,anatomy,physiology这些基本的课程。如果
上过pathology,histology,immunology,cell biology,genetics这些课程,那更是
巨大的plus。
这个program的外国人很少,其中一个原因是大部分program要求prereqs必须on us or
canada based transcript,也就是说即使已经在美国博士毕业,如果这些课是当年在

sperm. This is the 3rd time he got 0 count sperm. We don't know what is
