章很麻烦，耽误了2个多月，材料才得以从学校寄出。个人觉得申请算是比较顺利。把
我当时准备的情况跟大家说一下也许对将要申请的人有一点帮助。
我个人从这里寄出去的材料包括：
1.国内护士执照的公正，在这里网上找的公证处，在纽约，叫最佳公正，电话
7183212810
2.指纹卡，就在我住的地方的警察局办的，free
3.handbook上的申请表，贴一张照片在右下角（handbook链接：http://www.rn.ca.gov/pdfs/applicants/exam-app.pdf）
这一页有一项是填社会安全号，我当时没有就空着的，没有关系。
4.两张支票，一张办指纹的($51)，一张申请的($75)
5.写了一封信，表示感谢，一张纸，很客气的几句话
从学校寄出的材料：
1.中英文成绩单（学校有，我来之前到学校教务处办好的）
2.学位证书和毕业证书中英文版（来之前自己办好到学校教务处办好的，盖好了章）
3.课程描述9门（内外妇儿、精神科护理、生理、解剖、免疫、微生物，好像是这些，

再去上网看许多美国病人的评价，有许多spina bifida patient supporting groups
支持clinical trials。去那些美国人网站去看手术后病人怎么评价方粪spam他们网站。
许多方粪拿着这篇2009的综述，断章取义或英文太差，或别有用心。把一篇比较中性的
文章说成停用肖手术。

by spinal stenosis, a narrowing of spinal canal thus impinging on the spinal
cord.
It does not matter which school of medicine you believe, your mom need a
good diagnostic exam, preferably by MRI. After that, whether conservative
treatment or surgical treatment is dependent on the degree of compression,
because what you do not want is permanent spinal cord damage which is
irreversible. |

canal is 5 mm in maximal dimension. This is primarily due to disc material,
contributions from some ligamentous thickening and bony hypertrophic changes

从入院到产后4、5小时，妈妈在L&D room，然后转到普通病房，宝宝有小小的crib，病
床旁有折叠沙发，陪床的人可以躺躺，肯定不舒服了
大部分时候是两个人一间屋，如果妈妈比较虚弱，会单独一个房间，病房里两张床，这
种情况陪床的人可以躺得稍微舒服点
伙食：
美国的护士在学校里学的是病人肚子疼时给病人嚼冰块，这对咱中国人肯定不适合，可
以在入院时特别提出这点，要喝热水，不要冰块，护士会大字写在她们的本本上，交班
时会特意嘱咐下一班的护士
从入院到宝宝生出来，妈妈是不给吃的，陪床的人可以去医院的cafeteria，cafeteria
很小，大概有4、5样热菜，两个汤，吼咸吼咸，不加半瓶矿泉水根本喝不下，salad若
干，价格还可以，5、6刀能吃饱
产后几小时后，妈妈可以吃饭了，如果吃不惯医院的饭，可以自己带饭过去，有微波炉
可以热菜

这个诺贝尔奖级的成果最终得到了证实，朋友让我说说看法。
这篇文章是一个系列，见Journal of Urology December 2016 Volume 196, Issue 6,
Pages 1735–1740. 其实，已经有一个主评是密西根大学小儿泌尿科专家 见December
2016 Volume 196, Issue 6, Pages 1608–1609，认为这是一个值得学习的反面教例，
一点也看不出诺贝尔奖的样子。

possibilities (listed in the likelihood from higher to lower):
1. lumbar spinal stenosis seems able to explain every symptom he has, but it
was not shown on MRI you described. Another possibility is that arteries
supplying lumbar level nerve roots are abnormal. In non-invasive
examinations, CT/MR angiogram might help diagnosis in the latter case.

我女儿快三岁了，又漂亮又聪明，可是，她不会走路，而且永远也不会。她患有SMA (
Spinal Muscular Atrophy) 。这是一种genetic disorder，由于基因缺失身体无
法制造一种蛋白，严重影响她的运动神经元和所控制的肌肉，导致肌张力低下。SMA是
一种罕见病，发病率仅为万分之一，由于我和我老公都是携带者（我们都不知道，也没
有家族史），我们的孩子有四分之一的几率患病，我的女儿很不幸。

我生老大忍到了开7指，孩子位置不好，后背疼得不行上了Epidural，据说是CSE（
combined spinal-epidural），我在的是教学医院，麻醉是住院医生在一个很老的麻醉
师指导之下完成的，过程很顺利，麻药上完40来分钟开到10指，但是我在麻药上完之后
立刻就变成胸部以下全麻，完全无任何感觉，敲打，掐，估摸用刀戳都无任何感觉，腹
部完全无感觉。。。10指之后麻醉师把麻药降到Level 4，告诉我30分钟左右之后腹部
就会恢复压迫感，但是我过了3个多小时都无感觉，麻醉师也不明白为什么，就把麻药
彻底停掉，过了1，2个小时我才慢慢恢复感觉，回复感觉立刻又是无法忍受的背疼，麻
醉师坚持要再给我上Epidural。。。他说这次只给我再加最小剂量的epidural和止疼针
，不再加spinal，当时他认为我的腿部完全失去知觉可能是spinal导致的，他很有信心
我仍然可以无痛分娩。可是加了药上了epidural之后不到2分钟，麻醉师还没来得及转
身离开labor room，我的整个胸部腿部以及脚再次完全麻醉，这下麻醉师也懵了，立... 阅读全帖 |

发帖数: 206 | 30 请有经验的父母和医生们看看。宝宝出生过程中妈妈有发烧（羊水先破的）。当天就生
下来了。生下来的宝宝当时有发烧。医生就给了宝宝抗生素，很快就不发少了。blood
culture一直是negative, 做了CBC，说有个指标是50％（正常是小于20％）。这个指标
高有可能是宝宝有infection. 出生后到现在4天，一直用抗生素。第二天CBC有个指标
还是50％。第三天降到了30％。 第三天就正常了，是10％。
第二天的时候试图抽spinal fluid想检查是否有meningtitis, 结果试了两次。都没有
抽出来。医生本来是计划，如果spinal fluid检查是negative, 就打7天抗生素，出院
。现在没有办法检查spinal fluid, 医生就说保守点，弄14天的抗生素。
宝宝其他都所有检查都完全正常。现在就是比较纠结，是否需要打这么多抗生素。现在
宝宝已经没有感染了。大家给分析分析。 |

标 题: 宝宝需要14天的抗生素吗？

请有经验的父母和医生们看看。宝宝出生过程中妈妈有发烧（羊水先破的）。当天就生
下来了。生下来的宝宝当时有发烧。医生就给了宝宝抗生素，很快就不发少了。blood
culture一直是negative, 做了CBC，说有个指标是50％（正常是小于20％）。这个指标
高有可能是宝宝有infection. 出生后到现在4天，一直用抗生素。第二天CBC有个指标
还是50％。第三天降到了30％。 第三天就正常了，是10％。
第二天的时候试图抽spinal fluid想检查是否有meningtitis, 结果试了两次。都没有
抽出来。医生本来是计划，如果spinal fluid检查是negative, 就打7天抗生素，出院
。现在没有办法检查spinal fluid, 医生就说保守点，弄14天的抗生素。

spinal tap， 第三天night shift hospitalist告诉我MRI是normal，spinal tap不能
做， 因为病人on fragmin， 要等24hr。
我自己看了一下MRI， 这个病人有spinal abscess。 当天就手术了。
这个radiologist的误读让人很恼火， 可是这个fragmin更是火上浇油。 我不明白，
现在的hospitalist都用fragmin for DVT prophylaxis? Why not SCD + heparin for
high risk patients? 这个病人根本没有on SCD. |

T10就开始手术，基本上病人肯定要complain的。如果不是natural转c-section看来是
spinal给药不够，重比重.75 marcaine如果是腹正中切口病人在5'7"左右的尽量给到
15mg。重比重千万别怕给多，宁可多给。手里拿着neosynephrine不怕血压降，静脉全
开没问题。血压是可以自己调的，疼可是丢人。病人叫，护士不爽，做完了产科见人就
说病人喊疼弄得一上午全科人都知道了。
这种如果是epidural top up的，硬膜外马上给1.5% lidocaine来个10ml然后给75mcg甚
至100mcg fentanyl很快就没事了；如果是做CSE而spinal平面不够高，硬膜外临时给
top up就会很危险，很多文献说明确的有硬膜外从那个小洞往里面渗透，本来是喊疼还
凑合，现在弄成total spinal就彻底丢人现眼了。
如果带住院医的话，自己不能一直盯着就一定要强调15mg的重要性，不要怕多给，哪怕

这个诺贝尔奖级的成果最终得到了证实，朋友让我说说看法。
这篇文章是一个系列，见Journal of Urology December 2016 Volume 196, Issue 6,
Pages 1735–1740. 其实，已经有一个主评是密西根大学小儿泌尿科专家 见December
2016 Volume 196, Issue 6, Pages 1608–1609，认为这是一个值得学习的反面教例，
一点也看不出诺贝尔奖的样子。
肖氏手术指的是由腰到骶神经重建皮肤-中枢神经-膀胱反射通道，据肖传国的报道，能
达到80%以上的神奇效果，这就像江湖上的祖传膏药，5万多块一贴，在国内就做了两千
多，产值超过上亿对祖国GDP贡献也是很大的，当然都是那些贫病交加的家庭东筹西借
来的。由于疗效吹的很神，又有两千多中国人民做样本，所以引起了国际医疗界的兴趣。
