章很麻烦，耽误了2个多月，材料才得以从学校寄出。个人觉得申请算是比较顺利。把

家庭医生#1体检时查出颈部有小肿块 （大家annual check不要省,不要忘！），后作ultrasound发现一个1.1*1.9 cm肿块在甲状腺中间偏右，立马推荐了oncologist医生#2, 然后作biopsy，结果是 “Suspicious for Papillary Thyroid Carcinoma”, 医生#2说了由50%的概率是癌，要切除整个甲状腺。可是看到是50%概率，考虑到终生影响（终生吃药），而且他说不要做其他任何测试，心里没有底，感觉好像是骗钱是的。后来看文献，强力要求医生#2在做biopsy那家测试中心做了碘活性测试，结果是 “碘吸收正常，但是结合先前ultrasound的结果，所以结论是是cold,可能是Carcinoma”. 同时，本人联系了另一个oncology center , 人家根本不看，说你要见endocrinologist first, 你家庭医生怎么直接推荐了oncologist? 然后约了endocrinologist 医生#3， 再次作了biopsy，结果是“suspicious for Papillary Thyr... 阅读全帖 |

患者44岁，男，6年前因为肝癌做了肝移植 （符合米兰标准），之后无灾无病好几年。
去年2016年初发现轻微贫血，血色素12，因为当时在服小剂量阿司匹林，于是停服，看
看有没好转。托托拉拉到了年底，还是贫血，血色素10.5。医生说做个内镜。于是在胃
内发现一个肿瘤，内镜医生直觉是GIST。随后CT，但CT却没有异常。4月初第二次胃镜
，GIST依然还在，取样活检表明可能肝癌转移或者是非常罕见的HAC。第二次CT 报告大
小46x35mm，说比较第一次32mm变大了。可能第一次看片医生那天喝多了？肿瘤位置在
Fundus，非常接近GOJ。
AFP好久没有查了，因为肝上的情况一直很好，肝移植前AFP 100多，移植后AFP一直小
于2。上一次至少是2，3年前。也是疏忽了。现在AFP 254. 显然胃上这个要么是转移，
要么是HAC。
下周二医生还要安排一次PETCT，周三见外科医生，估计排除转移外就安排切除了。 求
这里的专家教授指点，这个情况如何是好？下面我贴上最近几个报告。

我岳母在美国被诊断查出cancer,医院出具了病理报告。她人现在已经在国内，为了节
省一些时间，需要把美国的病理报告提供给中国的医院作为参考，使她能够尽快地得到
治疗。由于涉及很多的专业词汇，我把并病理报告copy出来，希望专业的人士能够给各
翻译。

告拿回去，中国医生肯定大骂美国医生无能。那么多免疫组化，只有小部有用，大多就
为挣钱。
这个诊断除了说有癌，到底是不是内分泌，从哪来的，怎么解释染色结果，一笔糊涂帐。
你最好让他们send out for consultation.
从报告看,应该是转移性神经内分泌癌 (metastatic neuroendocrine carcinoma). 最
大可能是消化道,其次是胰腺,肺可能不大. 胰腺和肺,CT and MRI应该能见到.但消化道
有时原发灶很小,很难找到.

neuroendocrine carcinoma (神经内分泌癌) are 2 different things, and both
appear on the report. |

家庭医生#1体检时查出颈部有小肿块 （大家annual check不要省,不要忘！），后作
ultrasound发现一个1.1*1.9 cm肿块在甲状腺中间偏右，立马推荐了oncologist医生#2
, 然后作biopsy，结果是 “Suspicious for Papillary Thyroid Carcinoma”, 医生#
2说了由50%的概率是癌，要切除整个甲状腺。可是看到是50%概率，考虑到终生影响（
终生吃药），而且他说不要做其他任何测试，心里没有底，感觉好像是骗钱是的。后来
看文献，强力要求医生#2在做biopsy那家测试中心做了碘活性测试，结果是 “碘吸收
正常，但是结合先前ultrasound的结果，所以结论是是cold,可能是Carcinoma”. 同
时，本人联系了另一个oncology center , 人家根本不看，说你要见endocrinologist
first, 你家庭医生怎么直接推荐了oncologist? 然后约了endocrinologist 医生#3，

在北京时间10月23日晚TVB播出的《最佳男主角》访谈节目上，谢霆锋透露发现自己头部长有一块黑色的色块，“有一个瘤”，曾被医生诊断为carcinoma(皮肤癌)。霍汶希在微博中透露，目前已经“证实是良性”，有惊无险。

由黎芷珊主持《最佳男主角》邀来第三十届香港电影金像奖最佳男主角谢霆锋担任嘉宾。这次访问，主持黎芷珊特地拜访谢霆锋于北京的公司，听他细诉多年心路历程。

在最后的环节，谢霆锋透露“其实头上曾有一个瘤”。他在拍《十月围城》时因剪光头被发现头上有黑色的色块，近几个月发现色块有变大趋势，幸亏霍汶希心细及时发现，他及时就医。医生曾一度诊断为carcinoma(皮肤癌)。并已在近日切除，还缝了五针，留有一条5厘米的疤痕，因为手术缘故，特意剪短了头发。至于最后的化验结果尚未出来，但是自己很有信心能完全康复。

HBV-associated hepatocellular carcinoma 指出T:A >A:T在样本中是高频出现的。
在文章The genomic landscape of fibrolamellar hepatocellular carcinoma：Whole
genome sequencing of ten patients 也发现大量mutation.
在science的文章中用了UTUC的细胞列作为阳性对照。从上面发表的文章看出病毒也可
以引大量的类似突变。AA在肝脏引起的突变会和在肾脏一样？它们为什么不用HepG2，
hcv感
染的细胞，酒精处理过肝脏细胞，
AA处理过肝脏细胞系区分他们所测到的somatic mutation是AA在肝脏独有的。
