Maybe I need to make things more clear. Actually, I just matched to prelim
surgery program but NOT categorical program, so that I have to re-apply to
other specialty (I don't want to do surgery indeed) this year.
Do you still think the PD will feel bad for a prelim intern to interview for
other specialty? Please give me your idea. Tanks a lot.

You need to let the doctors know about your plan and come up with an
agreement. Some doctors may write more than one version of LOR, others may
not mention any specialties and the same letter can be used for multiple
specialties.

I just found this at DigiDoc's website:
Make the Letters 'specialty neutral' If you plan to use the same letter for
different speciaties- what I mean by this is that don't let the letter
specifically mention that you would be a great fit for 'pediatrics' or '
Internal medicine' - just let them say that you would be a great fit for '
your residency program'. Two reasons for this : a. The same letters can be
used for multiple specialties and b. This also avoids a mistake by ERAS guys

I found a mistake right after I reserved the LoR slot: I should assign the
LoR to all specialties but only put one specialty in the LoR. But I can not
edit it anymore. The instruction said i need to create a new slot in place
old one and let dean's office know that. But I can not delete the reserved
slot, how can I create a new one and in place incorrect one and let them
know? Also, I found when we apply for programs, we can assign the LoR to
programs, does it mean it is ok that I assign the LoR

Don't worry about this. The "specialty" field in LOR slot is for information
purpose only, and it will not be sent to any programs you applied. You can
assign a LOR to any programs, regardless of the "specialty" field.

You have to make up your mind soon. If you like IM most, Path as a backup
probably doesn't make sense (Path is more competitive). If you choose Path
first, you'll need IM as the backup. Many people apply for more than one
specialties. Just try to avoid applying more than one specialties at the
same place. Of course you won't tell the programs IM is your backup.

slots for residents each year has no big change.

time the biggest victims are Radio and Cardio. There will be 42% fee cut in
Echo study and 21% cut in stress test.

- Categorical positions (C) are in programs that expect applicants
who enter in their first post-graduate year to continue until they
have completed all training required for specialty certification,
provided their performance is satisfactory. Primary categorical
programs in medicine and pediatrics are designated by (M).
- Preliminary positions (P) provide one or two years of prerequisite
training for entry into an advanced position in specialty programs

There is no problem to apply for different specialties in the same hospial.
I got 3 interviews in the same hospitals, where I am working now.
Of course, you have no problem to apply for the same specialty in different
programs.

Each state issues medical license so a physician can practice medicine in
that state. Some physicians also take exams in a particular specialty to get
"board-certificated". Although no guarantee a "board-certificated" doctor
is more knowledgeable than other doctors in that field, it's an important
credential measure. As long as a physician has a medical license he or she
can see patients in that state. If not sure, choosing a "board-certificated"
doctor in that specialty is a safer bet.

Yoghurt, the reason your boss didn't want to set up connections for you this
year cold be that he doesn't want you to leave now, or he doesn't want to
influence too much for the residency selection process. However, as Dok
pointed out, the more you give, the more you will have. It's a human nature.
A chair I have done a research project with encouraged me to consider his
specialty, and implied a guarantee of a residency spot. Unfortunately this

be in a hospital with residency teaching and private practice. He said, we

to this program? This is a tough one.
They all want to know the career plan, and they like to train somebody that
can contribute not only to the program but also to the whole specialty.
Especially in those academic program, they want you to be a future leader.

they don't even mention the clinical training, all they want to sell to you

Just be aware that there are some drawbacks for preliminary surgery:
1) It doesn't count as the PGY-1 for specialties like IM, Peds, FM, and neurology;
2) It will use up one year residency funding, thus obtaining a spot of the
above specialties will be more difficult;
3) It will be very demanding and hard to arrange time for interviews in the
next application cycle.
Possible outcomes for preliminary surgery:
1) Match or promote to PGY-2 categorical surgery if such a position becomes
available;

If the residency of preventive medicine competitive, and what special
requirement needed? Never heard people talk about it.
Could anyone give an explaining? It looks like quite different from other
specialties, as it is not like a clinical specialty.
What kind of candidates they prefer?

it depends. for some nice people, I asked them to write for different
specialty. But for people that is not too close and less friendly, I didn't.
In fact, one person agree to write me different letter, but in the end, he
just wrote one, even I asked him again.So you could imagine it's probably
not a very strong letter)
Anyway, if you have enough people who are willing to write you good letters
(say five people in one specialty), you can ask them to write individually,
even one of them

Yes, you do need to apply a lot of program, IM, 100 is reasonable, assuming
you graduated more than 10yrs. If you are more fresh, less than 100 is OK,
and even some university programs may give you a chance. However, community
programs are still our major target.
You may not need to apply more than 100 for a second specialty. Remember,
dual specialty application means double your workload accordingly.

Personal statement for residency application doesn't need to be too dramatic
(a very convincing story will be good though). Basically you need to answer
these two questions:
1) Why do you choose this specialty?
2) What's your qualification for this specialty?
In addition, provide explanations for red-flags in your application, if any.

- Lack of commitment for the specialty

Is it absolutely not recommended to apply for different specialties (e.g.,IM
, FM, Peds) in the same institution? Does that affect getting interviews?
I know it might hurt if you go interviews for different specialties in the
same instituion. But does it have a negative impact on your chance of
getting interviews?

IM, FM, and Peds are similar primary care specialties. It's OK to apply them
at the same place. The chance for inter-department communication is small.
Even if they find out, you may say that you want to be a primary care
physician, and any of these specialties will suite your career goal.

In general the chance for inter-department communication is small. However
some small community programs could have the same stuff to handle residency
applications for multiple specialties.
1) If no one finds out, there is no effect on your application.
2) If they find out they may question your commitment for the specialties.

Paper published in China does not weigh much.
oriented specialty such as patho. Good if you research field happen to be
the same or close to the interest of the PD.
PhD also means that your graduation year >5. not good for IM.
overall impression:

FIND the answer:
A "categorical" position is one which offers full residency training
required for board certification in that specialty. A "preliminary" position
, in contrast, is a position offering only 1-2 years of training generally
prior to entry into advanced specialty programs. Many internal medicine and
surgery training programs offer preliminary positions in addition to
categorical positions. Transitional year programs are also considered
preliminary year training programs.

all such experiences are great and helpful to your future application and
interview, unless you don't understand what a treasure you got and how to
take advantage of them!!!
even FM or EM, they are useful for any other specialties--as OB is not for
you to learn a specific specialty and how to make dx or treatment, but to
understand US clinic system and communication between physicians with pts,
their family, nurse stuff and peers!!!

To be honest, Derm and Cardio are off the limits. Path requires a Ph.D and research background. I cant stand children yelling. So, Pedi is crossed out. The lesser good specialties have all been taken by the AMGs. You are the only specialty left for me................
Research is a stepping stone to come to the US. Glad I dont have to grow bugs again.....

I'm just curious. Didn't apply for either of these.
But your input is helpful to many of our friends here.
I met some transitional yr interns in the summer but didn't get the chance
to ask what their R-2 specialties are. So, usually which specialties they
will continue on?

Sure you may apply for some. Yes you need to say you want some surgery
training experiences, but not necessary for a surgery-related specialty. It
can also be used as PGY-1 for advanced specialties such as radiology,
ophthalmology, emergency medicine, and anesthesiology.

是。属于CP(Clinical Pathology).

1. I would agree 7 years as the limit, but you need to check DOS/USCIS
regulation to confirm. Normally it shall be enough to cover your residency.
2. If you drop, you drop. 2-year home country service is always there,
unless you get waiver.
3. one extra thing I confirmed with ECFMG about "Statement of Need"
a) if you stay with your program finishing your resiency, the letter is good
for the entire resideny, no renewal needed.
